I’m requesting approval to attend the 49th Annual Southwestern Clinical Pharmacy Seminar, Saturday &
Sunday, February 20-21st in Tucson, AZ.
This two-day conference, is held by the Arizona Pharmacy Association and the University of Arizona, is a
vital opportunity for me to keep up dated on the latest drug and disease state guidelines. I am willing to
