declare that I have submitted to Boyana Park Sofia EAD a request for signing a Contract for Rental of
Burial Plot and thereby I declare that:
1. Boyana Park Sofia EAD has provided me with:
1.1. information which identify them as administrator of personal data, namely: Boyana Park Sofia
EAD, registered in the Commercial Register under EIK 131507675, with registered seat in Sofia,
Vitosha District, 4, Boyana Park Str.
registered in the Register of Personal Data Administrators under № 0248069, according to
Certificate № 0248069/31.08.2009 issued by the Commission for Personal Data Protection;
1.2.the purpose of processing my personal data, namely: for the concluding of Contract for Rental of
Burial Plot; for internal and forecasting purposes and needs of Boyana Park Sofia EAD; for the
fulfillment of the rights and obligations under the Contract; upon the signing of the Contract for
Rental of Burial Plot and my burial in St./ Archangel Michael Memorial Park, for registering my name
in a public and freely accessible register of the individuals resting in the memorial park, which may
be stored in electronic version for the needs of the administration;
1.3. users and categories of users to whom my personal data may be disclosed, namely: my personal
data shall not be disclosed to third parties – individuals or legal entities, except when required by
law; my personal data will be processed only by authorized personnel of Boayana Park Sofia EAD and
individuals/ legal entities acting as ‘data processors’ under Art. 24 of the Personal Data Protection
Act (PDPA). My personal data may be provided to banking institutions, when this is required under a
registered pledge agreement for pledge of receivables signed between the respective banking
institution and Boyana Park Sofia EAD;
1.4.information about the compulsory or voluntary disclosure of personal data and the
consequences of refusing to provide it, namely: I disclose voluntarily my personal data in order to
enter into an agreement with Boyana Park Sofia EAD and a possible refusal to provide my personal
data will be deemed a refusal to enter into an agreement with Boyana Park Sofia EAD;
1.5. information about access and update rights on the collected personal data, namely regarding
my rights under Chapter 5 of the PDPA to: have access to the personal data relevant to me, to
request information from Boyana Park Sofia EAD at any time about the treatment of my personal
data, as well as about the purposes of the processing, the data type, the users and the categories of
users to whom the data is disclosed; to receive a notification in a comprehensible form containing
the personal data which is being processed, as well as any other information regarding the source of
the data, the logic of the automated processing of my personal data; to request from Boyana Park
Sofia EAD to cancel, correct or block the processing of my personal data, when it does not fall under
the provisions of PDPA, as well as to notify third parties about the respective data cancellation,
correction or blocking, except for the cases when the cancellation, correction or blocking will be
impossible or will require incommensurate efforts; to object to the processing of my personal data
by Boyana Park Sofia EAD, when there are legal grounds for that; to object to the processing of my
personal data for marketing purposes; to be notified when my personal data is to be disclosed for
the first time to third parties or when it may be used by third parties for marketing purposes and to
object to such disclosure. My access right and the other rights to which I am entitled can be
exercised upon submitting a written request to Boyana Park Sofia EAD signed by me or by a person
authorized by me through a power of attorney certified by a notary;
1.6. the term of storage of my personal data, namely: Boyana Park Sofia EAD will store, process and
manage my personal data for the term of the agreement;
2. I give my explicit consent, as per art.4, par. 1, line 2 of the PDPA, that Boyana Park Sofia EAD as
personal data administrator can store, process and manage all personal data I have provided
(including the data I am to provide in future) for the above mentioned terms and purposes. I also
agree that the present Declaration be stored in the documentary archive of Boyana Park Sofia EAD
for future PDPA compliance checks, regardless of whether a Contract for Rental of Burial Plot is
signed, together with copies of the other supporting documents required for the conclusion and
execution of the Contract for Rental of Burial Plot.
