Required Clerkships for the Fourth Year:
MD 900: Medicine Subinternship (4 weeks)
Because of the primarily ambulatory nature of the third year LIC, all students are required a
four-week, inpatient Internal Medicine Subinternship. At the completion of the Medicine
Subinternship students will demonstrate proficiency in the following core competencies
required for the care of the acutely ill adult patient:
1. Communication skills – students will demonstrate knowledge of local, national, and ethical
guidelines pertaining to patient physician communication. Students will demonstrate the
ability to communicate with patients and their family members. Students will also
demonstrate the ability to effectively communicate verbally and in writing with members of
the health care team. We will assess these skills by direct observation of the student by
faculty preceptors, residents, and other members of the healthcare team as reported on a
standard evaluation form, formative mini-CEXs, and in activities during the 4th year core
weeks.
2.
Coordination of care -- students will demonstrate the ability to contact all members of the
healthcare team and safely transfer care to other members of that team. Students will
demonstrate the ability to utilize and interact with consultants of multiple disciplines and
effectively cooperate with non-physician members of the health-care team. Students will
communicate with outpatient providers and community resources to facilitate a smooth
transition of a patient’s care between both the inpatient and outpatient settings. Students will
demonstrate an understanding of their patient’s plan of care both verbally and in writing. We
will assess these skills by direct observation of the student by faculty preceptors, residents,
and other members of the healthcare team as reported on a standard evaluation form.
3. Information management – students will demonstrate the ability to access and report
relevant clinical data on their patients. Students will demonstrate the ability to prioritize
tasks, document in a clear and coherent manner, and utilize both paper and electronic
resources to assist in patient care. We will assess these skills by direct observation of the
student by faculty preceptors, residents, and other members of the healthcare team as
reported on a standard evaluation form and during weekly student chief rounds.
4. Procedures – students will demonstrate the ability to explain the risks and benefits of
common bedside procedures to patients and their family members. Students will
demonstrate the ability to perform venipuncture, arterial blood gas sampling, nasogastric
tube insertion, and urethral catheter insertion. Students will demonstrate familiarity with
central venous catheter insertion and lumbar puncture. We will assess these skills by direct
observation of the student by faculty preceptors, residents, and other members of the
healthcare team as reported on a standard evaluation form as well as in planned simulation
sessions during the 4th year core weeks.
5. Professionalism – students will demonstrate the attitudes and behaviors of health care
professionals at all times, but especially in interactions with patients, family members, fellow
students, and all members of the health care team. We will assess these skills by direct
observation of the student by faculty preceptors, residents, and other members of the
healthcare team as reported on a standard evaluation form as well as in activities during the
4th year core weeks.
Requirements of the Medicine Subinternship:
3|Page
1. Students will work on an inpatient medicine service for 4 weeks.
2. Students’ usual work hours will be from 7AM to 6PM (weekdays).

with admissions and other responsibilities until 11 PM on average every 4th evening. Students
will not take overnight call in the hospital (in keeping with ACGME requirements).
4. Students will have primary responsibility for 3-4 patients at one time.
5. Students will participate in all required student educational activities for the medicine
subinternship including an academic presentation as determined by the medicine faculty.
Other Subinternships (4 weeks)
All other subinternships will require demonstrated proficiency in the five core competencies
outlined above. All other subinternships will share the five requirements outlined above as well.
Due to the differences in disciplines, other subinternships may have a different focus than the
medicine subinternship and may require demonstration of additional proficiencies. Students
may take a second subinternship as outlined below or students may take a second
subinternship in Medicine, MD 900, if desired.
MD 901: Pediatrics Subinternship
This subinternship focuses on the care of children on the pediatrics inpatient hospital service,
the well-baby nursery, and in the delivery suite.
MD 902: Family Medicine Subinternship
This subinternship focuses on the care of children and adults on the family medicine inpatient
hospital service.
MD 903: Medical/Surgical Intensive Care Subinternship (may fulfill the requirement for MD

crisis centers, and emergency departments. This subinternship can be adult, adolescent, or
child focused or some combination of these patient populations.
MD 909: Rural Family Medicine Subinternship
The subinternship in rural family medicine will be a combination of inpatient and outpatient
settings. Call may not be the same as for the other subinternships. It is expected that students
will work closely with an individual preceptor in his/her practice setting.
Other Required Clerkships

four, 12-hour shifts per week.
MD 912: Acute Care – Critical Care (2 weeks)
A required two-week rotation in an adult intensive care setting – students will spend time in an

Students are encouraged to follow patients from admission in the emergency department to
transfer into the intensive care unit. This rotation stresses the continuity of care through these
two settings. This rotation is offered by Wayne Memorial Hospital and Pocono Medical Center
in the Northern region.
5|Page
Interprofessional (IPE) Selective (2weeks)
All students are required to complete a 2-week selective in a highly functioning team. This
clerkship requirement addresses the need for physicians in-training to understand the roles of
all members of the health-care team and to function well within these teams. The IPE selective
requirement can be fulfilled through this IPE selective course (MD 915) or on a very rare
occasion at another site with preauthorization by one of the course directors(a request in
writing must be received a minimum of 30 days prior to beginning clerkship). This IPE selective
can occur in a number of different settings allowing students to foster individual interest so that
they can continue to develop the attitudes and skills necessary to practice medicine in a team
setting.
Other Required Activities
MD 999: Senior Seminar (1 week)
A required one week seminar that examines the issues of quality improvement and patient
safety, biomedical ethics, health policy and economics, medical informatics, cultural

collection of articles and videos within the constraints of one calendar week during the M4 year.
Core Weeks (2 weeks)

ALL TCMC clinical clerkships follow this block schedule and so clinical clerkships may be taken for
the first two weeks of the block or the second two weeks of the block for two week clerkships.
Four week clerkships may only be taken according to this block schedule.
Students taking clerkships at other institutions will find 1-2 week openings in their schedules
because of the difference in the timing of clerkships between outside institutions and TCMC.
Students are encouraged to fill these spaces with student designed clinical electives (MD 936,
MD 950, MD 955, MD 971, MD 977, or MD 979) or Electives from the Department of Basic
Science, the Department of Clinical Science, or the Department of Family, Community, and Rural
Health.
