OVERALL PURPOSE OF THIS CBO LINKAGES GROUP:


To identify key infrastructure or coordination processes and capabilities community based organizations
need for serving their communities effectively as part of the emerging “system” for better AD/dementia
identification and management, with particular emphasis on support to caregivers and families.
To synthesize key activities for CBO coordination/management into a CBO Checklist or algorithm template
that corresponds to a similar checklist developed for primary care providers.
MEETING OBJECTIVES:


Review and refine the CBO checklist and referral process flow (including a document that outlines the
respective roles of CBOs versus Clinics)
Establish next steps for fostering linkages between CBOs and Clinics
MEETING RESULTS:


Review and refine draft CBO Checklist:
o The group refined the standardized checklist to help guide CBOs. (under separate cover).
o The group also developed a draft process flow for optimal linkages between clinics and CBOs.
(under separate cover)
The Subgroup will develop a list of resources (see next steps) to support the checklist using the following
criteria:
o Preferred resource of CBO
o Evidence-based
o Statewide or community-wide organizations
NEXT STEPS
What
 Develop summary of meeting that clarifies goals
 Make changes to CBO Checklist as recommended by subgroup during the
meeting.
 Create a Resources Chart (page 2 of checklist) for completion by subgroup
via Google Docs. Send Link to Google Docs with summary.
 Add resources to chart on Google Docs
 Develop process flow for referrals based on feedback from subgroup
 Connect with Nan Just to obtain a one-page overview of Expanded Care
