1. Introduction to the Centre for Healthcare Redesign
The Centre for Healthcare Redesign (CHR) was established in 2007 as a key strategy to
support the growth of Redesign programs and therefore contribute to the Healthcare Triple
Aim1:
•
Improving the patient experience of care
•
Improving the health of populations
•
Reducing the per capita cost of health care.
The role of the CHR, as part of the Agency for Clinical Innovation (ACI), is to provide skills
and knowledge for staff across the healthcare system primarily in the areas of:

Redesign

Project Management
•
Change Management.
CHR offers capability development programs & resources which include:

2. CHR Diploma Program
The CHR has been delivering a state-wide redesign and project management diploma
program for health professionals since 2007. Through this program, the CHR supports the
goal to be the ‘go to’ place for clinician and consumer-led reform2 and aims to:
•
Enhance the health workforce’s capacity to undertake redesign projects within the
NSW Health system
•
Promote, develop and strengthen networks across functional and regional areas
•
Integrate patient-centred healthcare delivery.
The CHR Diploma Program is an intensive 20-week program conducted 3 times a year which
incorporates a mixed-method model of delivery to enable NSW Health staff to complete an
important workplace improvement project. It includes:

A comprehensive eLearning program
•
13 Face to Face educational sessions
•
Workplace coaching
•
A workplace redesign project
•
The award of a Diploma of Project Management for eligible participants.
Redesign Methodology
Using proven Redesign Methodology, frontline staff can identify issues impacting patient
experience then develop and implement sustainable change processes to improve the way
health care is delivered and experienced, benefitting patients, carers and staff.

Participants, projects and achievements
365 participants have graduated from the CHR Program, implementing over 200 projects in
the past eight years. These projects address a wide range of issues, including high profile
and state-wide initiatives such as:

Emergency Department (ED) Models of Care
•
Chronic Pain Management for Frequent Attendees to the ED
•
Individual Patient Special for Patients with Delirium and Dementia
•
Hospital in the Home for Patients with Cellulitis
•
Emergency Surgery Guidelines
•
National Emergency Access Target (NEAT) Projects

Project Idea
Projects best suited to the Redesign Methodology are improvement projects, where staff
identify a high priority and measurable problem to solve in their organisation.
Initial Formulation
Once the issue / problem area is identified, staff should start defining the project. This
includes determining what the objective is, who will manage the project, and how the project
team will be organised. The application form offers a list of criteria to think about prior to
starting the project.
CHR Application – Who is eligible to apply?
Staff who regularly manage projects or wish to make significant workplace changes, but do
not have any formal training on project management or change management, will greatly
benefit from attending the program. Participants are nominated from each Local Health
District / Specialty Health Network (LHD/SHN) on the recommendation of the local Executive.
Staff can apply individually or in teams. We recommend that the project team comprises of at
least 2 participants, including at least one member of each area impacted by the change.
For example, if the project addresses issues for patients with a mental illness when
presenting to the Emergency Department, the team should include at least a staff member
from Mental Health and one from the Emergency Department.
The application form must be reviewed and approved by your Manager, your Project
Sponsor and your Redesign Leader prior to being submitted to the CHR.
If you are thinking about applying but
are unsure of the suitability of your project or
the commitment needed for the program,
it would be a good idea to talk to your
Redesign Leader or to a past participant.

CHR Diploma Program
The Diploma Program provides the learning component to set up a successful project. The
program takes participants through the different steps of redesign project management, from
initiation through to evaluation.
The program requires a considerable time commitment and we recommend that
participants are backfilled for 3 to 5 days a week during this phase (dependent on the scope
of the project – large projects require full time commitment to underpin success). Redesign
Leaders provide valuable support in coaching participants. The main outputs of the program
are a strong implementation plan and tools for successful change management.
Project Implementation
Succesful Project implementation is key in achieving project objectives. During this phase,
participants are accountable to their organisation for the successful implementation of their
projects.
This phase is also highly time-consuming and we recommend that participants are backfilled
until the changes are part of the “business as usual”. Redesign Leaders continue to provide
coaching. The Project Sponsor plays a key role in accelerating the implementation.
Objectives Met - Evaluation and Graduation
Objectives are usually met 4 to 12 months after the end of the CHR Diploma Program. This
can vary significantly depending on many factors such as the scope of the project, the
availability of resources (including participants’ time) and the involvement of the Project
Sponsor.
A Graduation Day is organised 4 to 6 months after the program, where successful
participants present their achievements and obtain a Diploma of Project Management.
Continuous Improvement
The redesign process is part of continuous improvement activities. Projects, goals and
objectives should be constantly revaluated to ensure the systems meets the demand and
progresses towards achieving the Triple Aim:
•
Improving the patient experience of care;
•
Improving the health of populations; and
•
Reducing the per capita cost of health care.

4. Course Structure
The content of the program is based on Project Management, Change and Redesign
Methodology. Participants who successfully complete all the course requirements and
manage their improvement project until completion are eligible to receive a Diploma of
Project Management.
The program consists of 5 phases held over a 20-week period, each of these including:
•
Online learning modules, providing participants with the knowledge to support their
practical learning sessions (4 to 8 hours of work for each session, to be completed
prior to attending the relevant Face to Face workshop);
•
Face to Face learning workshops, designed to give participants the opportunity to
develop the practical skills needed to undertake specific tasks related to each stage
of the project;
•
Project work where participants focus on developing their project and putting their
new skills and knowledge into practice;
•
Reports to submit to the CHR and project updates to present to the group (from
Diagnostics onwards).

This session includes the first steps of setting up a project, from defining
objectives, to developing a communication plan. Participants will be able to
complete part of their Project Management Plan during the sessions.
These days cover a range of diagnostics tools such as theory of constraints,
process mapping and data analysis. Participants will use the most appropriate
tools in the workplace to identify the issues’ root causes to address.
During these workshops, participants will learn and try different techniques to
identify solutions and prioritise them. One of the 3 days is a site visit to Baxter
Healthcare, introducing Lean Thinking.
The implementation session develops the principles of Accelerating
Implementation Methodology (AIM). This methodology delivers strategies to
ensure successful change management and project implementation.
This session covers the closing steps of the project, including how to evaluate
project outcomes, sustain them and spread the success to other services or
organisations.
Participants attend 2 check points with ACI staff to monitor project
implementation and discuss successes and / or potential issues arising during
this phase.
A Graduation Day is organised 6 months after the end of the program. Team
Members, Sponsors and Redesign Leads are invited to support participants and
learn from their projects outcomes and experience of the program.

5. Pre requisite
1. Participants wishing to apply to the program need to have an improvement project
approved by their sponsor and backfilled time to manage it.
2. Participants are required to have attended an Accelerated Implementation
Methodology (AIM) course in the 2 years prior to starting the course. For participants
who didn’t have the opportunity to attend an AIM course, or wishing to review key
concepts, a one-day workshop has been scheduled before the course starts.

7. Cost
The NSW Agency for Clinical Innovation will cover all costs related to the delivery of the face
to face teaching and eLearning modules as well as the certification costs for the diploma for
each participant.
Local Health Districts will need to cover all costs associated with travel and accommodation
for nominees to attend the program.

8. Key Program Roles and Responsibilities
Program Participant
The project manager and team member/s for the local team is in charge of project activities
during the 20-week program and accountable for following the methodology for the life of the
project, including the implementation and evaluation of solutions.
Program participants are required to:
•
Attend all Face to Face sessions including the Graduation Day
•
Have backfilled project time. This is a fundamental aspect of the course as the project
worked on while completing the program will require significant commitment in their
workplace
•
Complete all pre-course requirements and deliverables in a timely manner
•
Share ideas and learning to create a mutual support network for change
•
Implement their project solutions.
The project team will have the support of their organisation, maintain open communication
and network at a senior level within the organisation.
If they are not in a project role, participants should be backfilled 3 to 5 days per
week (according to project scope) to undertake project tasks such as workshops,
data collection and development of deliverables.
Project Sponsor
The Project Sponsor has the authority to make the project happen. Their role is to legitimise
and demonstrate ownership for the change. As such, the Project Sponsor will work closely
with the project manager to ensure:
•
Appropriate scope and goal setting in line with organisational strategic goals
•
Allocation of resources and project budget
•
Regular scheduled monitoring of progress and approval for project milestones /
deliverables
•
Senior level commitment and sponsorship by senior executive teams
•
Facilitation and resolution of issues at senior levels
•
Reinforcement of expected new behaviours.
In support of their project team, sponsors are required to attend the following
sessions:
- 1st face to face day (project initiation)
- 1st sponsors’ webinar
- Final Graduation
More sessions will be scheduled through the course to enable Sponsors to
support their project teams.

Centre for Healthcare Redesign Course Facilitators
The CHR course facilitators create:
•
A challenging experience where you will learn and develop new capabilities in
redesign, change and project management
•
An opportunity to learn from, and be coached by, recognised experts in the redesign
field (coaching and mentoring will be provided by ACI program facilitators and
Redesign Leaders)
•
A learning culture supported by coaching and online educational resources and
templates
•
Opportunities for knowledge transfer.
Redesign Leaders
The Redesign & Innovation Network consists of 27 experts in Project and Change
Management across all Local Health Districts and Networks. The Redesign Leaders play an
active role in supporting the CHR Program, in particular by:
•
Identifying optimal projects and participants to attend the program
•
Coaching and mentoring the participants and projects from their LHD/SHN
•
Contributing directly to the CHR Diploma Program delivery by facilitating individual
sessions with participants and providing expert feedback after their presentations.
•
Facilitating networking within and across clinical and geographical boundaries.

1. Application Process
To apply to the CHR Diploma Program, candidates are requested to submit a project brief.
Candidates are required to contact their Redesign Leader in the first instance as there may
be a specific local application process applying to their organisation.
If you are unable to contact the Redesign Leader, enquiries can be made directly to the CHR
Course Manager, Estelle Marque on Estelle.Marque@aci.health.nsw.gov.au.

To ensure an optimal learning environment, each course will be limited to a maximum of 12
projects or 25 participants. Applications will be assessed against the criteria outlined in the
application form. These include the appropriate number of participants attending the program
(ideally 2 per team).
Team participation
Participants are encouraged to submit a group application with some of their team members.
To comply with the Diploma of Project Management requirements, there is a limit of 4
participants working on the same project attending the CHR Program.
Please note that to obtain the Diploma of Project Management, all the team members need
to attend the Face to Face days, complete their eLearning individually and play an active and
equal role in the project implementation.
By applying to the program, all participants agree to share their project
information with the rest of the participants. They also agree that all information
shared is to be used for learning purposes only.
