Classroom Observation Forms
The observation forms that follow are designed help preservice, student, and beginner teachers
focus on what is taking place in a classroom and assess a teacher’s performance based on the
involvement of students in a lesson. Ask other teachers if you can use the form while observing
their lessons and videotape and assess your own performance.
1. Observing students. Observe an individual student for a class or day and complete the
form at the end of session.
2. Teaching time. During each time segment, place a mark in the appropriate box every time
a teacher engages in the activity. At the end of the class period tally the marks in each
column.
3. Quantitative classroom mapping. Complete a seating chart. Mark the box each time a
student actively participates in the lesson.
4. Qualitative classroom mapping. Complete a seating chart. Place an appropriate code
marking in the box each time a student participates in the lesson.
