PCA JOB DESCRIPTION JOB TITLE: Care Coordinator - Emergency Services (ER) JOB ACCOUNTABILITY & OBJECTIVES: This non-exempt position works under the supervision of the Chief Clinical Officer (CCO). This position combines several skill sets for organizing, data entry, maintaining files and tracking clients in crisis situations. This position maintains data entry for court tracking, hospitalizations, and referrals for alternative services. This position assesses the strengths and needs of clients and their families; plans, provides access and coordinates holistic continuing care and services for clients and their families for prevention and diversion of hospitalization. This position serves as an important liaison between clients in need of emergency services and aftercare. This position also performs the processing duties of emergency services, including contacts and billing. This position is expected to maintain positive provider relations with community organizations and hospitals, and provide internal and external customer service consistent with Professional Counseling Associates’ (PCA’s) goal of excellence and professionalism. RESPONSIBILITIES: ESSENTIAL FUNCTIONS: Maintains data entry and tracking of the following: SALS from emergency contacts (screenings/phone); court docket. Coordinates with Clinic Support Specialists to assure Emergency Services SALS are entered correctly by checking Weekly Posting Sheet. Maintains tracking of clients for inpatient and other alternative care services. Establishes and maintains files for emergency services clients and court cases. Determines client status within agency and notify proper clinics of court requirements and ER contacts. Assists with billing and monthly report for indigent care contract Distributes inpatient information (i.e., discharge summaries, evaluations) to the proper clinics for active clients and/or Court ordered cases. Assists Daytime Screener with information as needed and/or requested on a daily basis. Coordinates records, appointments and billing for court ordered clients, and contract forensic evaluations. Coordinates screening requests by gathering information from requesting party, and then dispatch screener as needed. Obtains and forward records from charts as requested. Forward records to ASH and other facilities as appropriate (includes knowledge of use of ASH shared drive) – pending appropriate release of information. Page 1 of 3 Provides phone crisis contacts with clients, to aid in their ability to access community resources or emergency services. Maintains appropriate documentation of interventions and services provided to clients, as well as confidentiality as outlined by policy and law. Certified Mental Health Paraprofessional: a. When providing any clinical service, work under the direct supervision of a Mental Health professional as a MHP service extender. b. Successfully completes prescribed and documented courses of initial and annual training sufficient to perform all tasks assigned by a mental health professional. c. Acknowledges in writing (by signing this job description) that all mental health paraprofessional services are controlled by client care plans and provided under the direct supervision of a mental health professional. Coordinates and prepares referral correspondence to appropriate mental healthcare facility. Interacts with inpatient facilities for follow-up and other needed information. Obtains authorization from other mental health facilities to screen their out-ofcatchment-area residents and clients, and monitor SAL entries to make sure correct dollar amount is billed. As needed, provides back-up for the Administration Support Specialist –Receptionist when this person is absent. NON-ESSENTIAL FUNCTIONS: Performs other duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of DSM-IV diagnoses and interventions used in the treatment of disorders. Knowledge of community agencies and resources including medical, dental, housing, education and mental health. Ability to respond calmly and effectively in a crisis Ability to establish and maintain effective communication and working relationships with clients, families, law enforcement, court officials, staff and other professionals and providers, and the community, in general. Ability to accurately input computer data, develop and print reports from the computer; ability to establish and maintain effective working relationships with clients, staff and the community. Ability to organize and keep records in a systemic manner. Page 2 of 3 EDUCATION AND EXPERIENCE: Must be a qualified mental health paraprofessional as defined by State Standards. Past experience in mental health and utilization of community resources is preferred. Must have one of the following credentials: Bachelor’s Degree in Social Work, Psychology, Counseling or a related human services field (or equivalent). Must complete training for certification as a mental health paraprofessional (MHPP) if not certified as an MHPP. SPECIAL CONDITIONS: A criminal background check is required every 5 years. Adult maltreatment and child maltreatment registry checks are required every 2 years. A drug screening, TB test and safe driving record are required. CPR and First Aid Training are required.
