Induction Library Media Specialists will submit PD 7 ADEPT Reflection to Mentor/Observer/ Assistance Team Chair within seven (7) days of each observation Mentor and Assistance team Chair will conduct feedback conference on observation and reflection within five (5) days of receiving reflection PD 1 Long-Range Plan (Media Specialist) Media Specialist submits Long-Range Plan to the team chair Veteran Library Media Specialists Observations Induction Contract Educator will conduct observations of approved veteran educator PD 2, 4, 5, and 6 Participate in Interviews Assistance team members conduct interviews and complete documentation PD7 Fulfilling Professional Responsibilities School Leader and/or other designee submits Professional Performance Description to the assistance team administrator PD7 Professional Self-Report Teacher submits Professional Self-Report to the assistance team administrator Documentation Verification Meeting Assistance team administrator must hold a meeting with the school leader to discuss recommendations/ improvements. Initial/Preliminary Evaluation Conference School leader and/or assistance team members must hold initial/preliminary evaluation conferences. All preliminary information (including improvement plan documentation) must be delivered to Human Resources on or before 5:00 pm. PD 3 – Media Specialist reflections must be submitted to the team chair within seven (7) calendar days following each evaluation.
