SUMMER VACATION CARE 2015/2016 For primary school children (K – Year 6). Bookings close on Friday 4 December 2015. Booking forms attached to this program must be completed and submitted for your booking to be processed. All payments must be made by Friday 11 December 2015. Non-payment by the agreed date will result in the place being offered to the next family on the waiting list. Bookings will not be processed for those families with any outstanding amounts owed from after school care or past vacation care programs. The preferred payment method is online: https://online.cityofsydney.nsw.gov.au/ChildCare The alternative is to pay at the Community Centre front desk (please discuss process with the children’s program staff) or by cheque. Cheques must be made payable to “The City of Sydney Council”. Fees are charged for every day that you book, whether your child attends or not. Excursion fees can be reimbursed for an absent day if you provide a doctor’s certificate and call the service no later than 9am to advise your child will be absent. Families not claiming CCB or in the process of being assessed for CCB will be required to pay the full amount upfront. Families can access Child Care Benefit (CCB) to subsidise these fees as well as the Child Care Rebate (CCR) which can be allocated towards the payment of fees. To receive CCB or CCR you need to register with the Department of Human Services (DHS) on 13 61 50 between 8am and 8pm, Monday to Friday or 13 12 02 for information in languages other than English. Purpose of Collection of Information on this form: To be considered for a place at our vacation care program for primary school aged children. Intended Recipients: Relevant City of Sydney staff concerned with the provision of children’s services. Supply of Information: Not legally required. However if you don’t give us the required information, it will affect our ability to allocate priority to you, or to contact you should a place become available at the centre. Program Dates Thursday 17 December - Thursday 24 December 2015 Monday 4 January - Tuesday 27 January 2016 Cost $40 per child per day plus additional excursion costs Hours 8am – 6pm The program closes at 6pm. Penalty fees of $1 for every 1 minute after 6pm apply, with a $10 minimum for late pickups. Bring Hat Lunch prepared ready to eat, healthy snacks and drinks Wet weather gear If your child is allergic to sunscreen please send your own What to Wear Please ensure that your child wears appropriate clothes to the centre each day, please NO SINGLETS or singlet dresses. Shoes that completely cover the toes and protect the foot should be worn - no sandals, ballet shoes, or thongs. If your child is dressed inappropriately for the day’s activity/excursion or is not dressed appropriately to meet our sun smart policy you may be asked to collect your child from the centre or to provide appropriate clothing. Signing In & Out All children must be signed in and out by a parent or guardian and visual contact must be made with a member of staff. Absences & Sick Children If you have booked but cannot attend please notify us as soon as possible. Sick children are not permitted to attend the centre as they may spread infection. If you need us to administer medication to your child, you must complete a Medication Authority Form which you can get from us. Swimming Excursions Children should be comfortable in the water, be able to swim and dress themselves unassisted. Additionally they should be 6 years of age or older. 5 year olds and nonswimmers will be provided shallow water options. Valuables We are unable to take responsibility for or secure valuables (including toys, electronic games etc.) or money children bring to the centre. Children are encouraged to not bring any such items to the centre. Spending money is optional and is the child’s responsibility and we cannot guarantee that the program will have time to stop so that any child can buy their lunch, buying something from a shop cannot be guaranteed. Risk Assessments Risk assessments have been conducted for all excursions and have been communicated to all educators involved in the program. Families can access these upon request.
