Consent form for the use of a taxi for wrap-around
(This form should only be completed by parent/carer of children using our wrap-around service)
In line with the National Minimum Standards, Cylch Meithrin Llanilltud Fawr have
undertaken a number of measures to ensure your child’s safety when using the taxi
service for wrap-around to

The Cylch has a contract with Llantwit Major Cars / Llantwit Major Travel. The Cylch has
ensured that the vehicles used are both road worthy and insured and also that all drivers are
CRB checked. (Contact information and a copy of this contract are available upon request)
This contract details that children will be collected at 12:45pm to arrive at the school for
12:55pm in time for the start of the afternoon nursery session, and for afternoon
children to be collected at 12:55pm and taken to the Cylch for our afternoon session
at 1:15pm
With regard to safe conduct, this contract also includes information and insurance
details for the vehicles that will be used to transport the children and a list of the
name of named drivers including their CRB disclosure numbers.
The Cylch have purchased a number of age appropriate booster seats that will be
used in the taxi and stored in the Cylch. A copy of the safety information for these
seats and the seats themselves are available to be viewed by parents/carers at any
time.
Should you wish your child to use a car seat, this much be purchased by you and
brought to the Cylch each morning, this will then be left at the school for you to
collect.
I give my permission for my child to travel in the taxi(s) arranged by Cylch
Meithrin Llanilltud Fawr providing the terms outlined in this agreement are
fulfilled
