job description job title: Staff Nurse (Split Children/Neonatal role) accountable to: CEO responsible to: Director of Care hours: 37.5 salary: £26,202 to £30,375 job summary: To provide holistic care and support to children and babies with life limiting and life threatening conditions and their families. The post holder will be required to provide care across a range of settings in the community and at the hospice. The job includes regular evening, weekend and overnight working. The post holder will have experience working with both children and neonates, and will work alongside our Neonatal Coordinator to develop the neonatal service offer to families from 20 weeks gestation. The role will also include working with children up to the age of 19 years as part of our nurse therapy led team. The post holder will work in collaboration with other services providing care and support for the child and family. key responsibilities: To ensure that holistic care is family centred, needs led, evidence based and meets all statutory requirements. To assess, plan, implement and evaluate care plans to meet the diverse and individual needs of children and families, reporting these back to the Care Coordinator, and Referral Panel as appropriate. To contribute to the care management of families as directed by the Care Coordinators. This includes managing referrals, ongoing and annual reviews and transition plans. 1 To be the named Neonatal In-reach Nurse to lead multidisciplinary working across the local neonatal units, community services and other hospice providers. To provide an in-reach and liaison role, becoming increasingly embedded within the work of neonatal units and associated services. To lead shifts as required. To ensure that all risks associated with care delivery relevant to the care setting are managed effectively and that all incidents are reported promptly following the policy and procedure. To participate in the on going development of our palliative care services. To participate in quality assurance and practice development activities, including audit, benchmarking, service review and reflective practice to ensure that the highest standards of care are maintained. To work proficiently and effectively within the relevant legal, ethical and professional frameworks applicable to children’s palliative care practice in the community and in the hospice at all times, including the NMC Code of Professional Conduct. To share professional expertise with Care Team colleagues, providing support and training where appropriate. To be responsible for maintaining and updating your own professional practice by actively participating in reflective practice, action learning, appraisal, management and practice supervision and to be a full and willing participant in opportunities for learning To act as a mentor, with recognised qualification, to Care Team Members and students and participate in the education and development of colleagues. To undertake practice supervision To act as an ambassador for the organisation and to maintain positive and effective communication with other agencies involved with families receiving our care services. To attend discharge planning, review and debrief meetings as appropriate. To participate as a team member in other activities which complete the care cycle, i.e. laundry, meals, ad hoc light domestic duties and outings with the young person and their family. 2 To undertake any other duties as are within the scope, spirit and purpose of the job, the title of the post and it’s grading, as requested by your line manager or their higher level of authority. The managerial and clinical philosophy of the Forget Me Not Children’s Hospice care services is based on an interdisciplinary ‘generic role’ approach. All staff, regardless of their grade or discipline are required to participate according to this concept. All staff must be able to project the philosophy and concept of our hospice care services.
