Checklist for Submission of Audition forms/DVD
 Submit the completed Audition Form for EACH child who wishes to audition.
 Have a MUSIC LEADER/TEACHER complete the form.
 Vocalize children and place range on the Audition Form. Do not record this.
 Please rehearse the children before recording their two selections.
 Record ALL video/audio auditions on one DVD. At the beginning of the recording please
use the following procedure:
1. State the child’s name, grade, and the name of their Church/School.
2. Re-record if there are any mistakes. Only include one “take” on the DVD.
3. Remember: Choose two songs, one accompanied & one unaccompanied.
GIVE STARTING PITCHES FOR UNACCOMPANIED SELECTION.
 LEADERS: Please number your final forms in the order that they appear on the DVD
and mail in ONE envelope so that they will reach the Church Music Department before
November 15, 2016.
