PEDIATRIC RESIDENCY TRAINING PROGRAM OREGON HEALTH & SCIENCE UNIVERSITY DOERNBECHER CHILDREN’S HOSPITAL PROCEDURE DOCUMENTATION POLICY General Principles Documentation of procedures is necessary for: Hospital privileging American Board of Pediatrics Certification Residency Program review and accreditation All procedures are to be documented on the E-Value Procedure Log website: http://www.e-value.net Residents will receive a login in password from the Program Coordinator at intern orientation. Procedures to be tracked by program: Basic Life Support (BLS) Pediatric Advanced Life Support (PALS) Neonatal Resuscitation Program (NRP) Documentation of completion of these programs will placed in the Resident Competency Portfolio by the residency staff Procedures to be tracked by resident These procedures MUST be tracked using the E-value Procedure (PxDx) Log: Endotracheal Intubation Umbilical artery and vein catheter placement Lumbar puncture Circumcision These procedures SHOULD be tracked using the E-value Procedure (PxDx) Log: Reduction and splinting of simple dislocations/fractures Bladder Catheterization Suturing of lacerations Pelvic (GYN) examination Removal of foreign body Incision and drainage of superficial abscesses Chest tube placement -2 Central venous catheter placement Arterial line placement Paronychia/ingrown nail treatment Bone marrow aspiration Arthrocentesis (joint aspiration) Paracentesis Procedures ideally should be logged as soon as possible after the procedure and logs updated at a minimum semi-annually. Procedure logs will be reviewed semi-annually with residency program leadership. Reviewed and approved by Residency Education Committee 12/8/2009
