Victory Home Medical is a contracted provider for Amerigroup and is also willing to work with the other
Bayou Health plans on a case by case basis. She also accepts private insurances. Moms can pick up
pumps from her or receive via Priority mail to their home. Currently providing Drive brand pumps, but is
looking into other options such as the Medela PIS. Requires a prescription stating need for “Double
electric breast pump.”

United Healthcare (UHC). Target pharmacy will provide an Ameda Purely Yours double electric pump
once mom’s benefits are verified. A prescription for double electric pump from mom’s OB is required.
Stock is often an issue at local Target pharmacies.
OR…….
Prenatal and postpartal mothers can call UHC at 1-855-406-7867 and request their pump be mailed
directly to their home. Expect a 2-3day average on shipping.

Gilsbar 360, PHCS, Web TPA, and Verity. Begin by calling or filling out the request form on the website;
after which, Airway will contact you and your provider. Airway offers a variety of pumps and
replacement parts at little to no out of pocket cost depending on your insurance benefits.
