Functionality/Service Overview
ShareMyCare
ShareMyCare is an N3-hosted ‘web-based’ data entry portal launched in
2002 to support the sharing of summary handover of care plans between
the GP Practice and the local health and care community including End of
Life Care, Long Term Conditions and other relevant Special Patient Notes.
The solution has been developed from its introduction, now supporting
integrated two-way communication between those involved in the care of
the Patient and has continued to meet the mandatory gap between other
solutions whereby the identification of the plan type and Patient pathway
need is flagged in summary to non-clinical staff at the receipt of the
Patient’s episode of care, and more detailed summary of information that
meets ISB standards to support appropriate clinical care. An example is
the systems use in NHS 111 services whereby an End of Life Care Patient
wishes not to be admitted to Acute care however without prior flagging to
non-clinicians of this, the Patients symptoms would likely initiate an
automated dispatch of ambulance from our integrated NHS Pathways
Clinical Decision Support software. ShareMyCare also provides
integrations into TeleCare and TeleHealth.
GP Practices do not wish to have systems that duplicate process and to
date a considerable amount of functionality has been introduced to
prevent this however the extension of ShareMyCare into its existing
environment to meet the Authority’s requirements for Data Entry Forms
overcomes the greatest, and remaining frustration in the information
sharing process for GP Practices, whereby manual or mail merged transfer
of information from the GP Principle System occurs to support sharing that
is mandated by NHS England across service boundaries but strategically
the end-to-end service requirement has not been met. Through extending
ShareMyCare to support both individual and bulk instantiation of dataentry through an integrated method (using the Interface Mechanism), we
support GP Practices in meeting their Data Entry Form requirements and
overcome barriers that create unnecessary duplication in GP Practice
administration.
Current Integrations with Principal Clinical Systems:

ShareMyCare provides current integration through use of Word templates
that are manually imported into the Principal Clinical System, these
template, following an export process from the Principal Clinical System
can be sent to ShareMyCare where they are automatically imported and
create or update the patient record.

ShareMyCare is a centrally hosted browser based solution which provides
an always on service as standard. Because ShareMyCare is also used within
