Background:
One Hackney and City is an innovative model to integrate care more
effectively for patients with complex needs and those in the last years of life. The
model is designed to improve the outcomes and experience of care for patients
and their carers and families by facilitating the integrated working of different
services. This is an exciting opportunity to be involved in a flexible model of care
using learning and experience to make a positive difference for Hackney
residents.
Job Summary:
The focus of this role is to ensure that the Care Co-ordination team is effectively
supported to provide safe, high quality support to patients and their families.
The One Hackney and City Clinical Manager will provide day to day line
management for the team of One Hackney and City Care Co-ordinators (5
people) and Administrators (3 people).
In addition, the Clinical Manager will provide clinical supervision to the Care Coordinators, in support of their roles in providing critical co-ordination and liaison
between patients, families and local providers to ensure that effective packages
of care are developed and provided for people with complex health care needs,
mainly elderly and those in the last years of life.
The Clinical Manager will also hold clinical responsibility for a defined patient
caseload, in support of the Care Co-ordinators.
The post holder will be a qualified clinician, either a registered nurse or allied
health professional, with significant experience of directly line managing teams
and providing individual clinical supervision.
In particular, the post holder will be experienced in the HR functions associated
with line management including undertaking staff appraisals and objective setting.
The post holder will be adept at providing group and individual management
support, together with the ability to challenge established processes and the
unblocking of obstacles to change.
The Clinical Manager will need to have an evidenced commitment to collaborative
and integrated working and to playing an active role in supporting liaison with
other health and social care professionals and partner agencies.

Key Responsibilities:
Clinical
1. To support the Care Co-ordinators to provide a central co-ordination role for
patient care planning, to keep patients stable and safe at home, and to avoid
unnecessary hospital admissions;
2. To be responsible for the provision of clinical supervision of the Care Coordinators. The post holder will provide regular clinical supervision and
professional support to each Care Co-ordinator to support critical reflection
and support safe practice;
3. To support the team of Care Co-ordinators to work effectively with vulnerable
patients, resolving problems, supporting patients to access services and
support required, and by ensuring that appropriate action is taken when
patients need help. This may include accompanying Care Co-ordinators on
visits to patients’ homes; however these visits take place rarely;
4. To facilitate smooth and planned discharge and handover between care
settings across the health and social care system, and be responsible for
facilitating inter-agency communication and support.
5. To manage a small caseload of patients with complex needs, as required
having reviewed Co-ordinator workload;
6. To support the provision of feedback to GP practices by Care Co-ordinators,
resolving problems and escalating actions as necessary, providing advocacy
for patients wherever necessary;
7. To facilitate and ensure the effective delivery of patient-centred, personalised
health and social care plans for patients, monitoring progress and reporting
outcomes to the patient’s GP and Community Matron;
8. To attend, support and contribute to practice monthly MDT meetings, in
conjunction with Care Co-ordinators, when individual One Hackney and City
clients are to be discussed;
9. To attend and contribute to quarterly Quadrant MDT meetings (facilitated by
the Tavistock and Portman) – these meetings build on monthly practice-level
MDT meetings and address the development and education needs of
practices and other members of the multi-disciplinary team in relation to
delivering integrated care. These meetings also identify any service gaps for
improving care for frail patients.
10. To liaise with GP practices to ensure that GP practice registers of vulnerable
patients who are part of the One Hackney and City programme and the Frail
Home Visiting enhanced service contract are kept up-to-date and to advise
practices where care plans need updating, as required;
11. To provide support to the named acute sector Care Co-ordinator in liaison with
the acute sector if One Hackney and City patients are admitted into hospital
and to ensure that any vital information is shared with acute sector colleagues
to support the development of the patient’s treatment plan and to avoid any
unnecessary interventions or investigations;
12. To provide support to the named acute sector Care Co-ordinator in liaison with
the acute sector on discharge planning when One Hackney and City patients
are admitted into hospital or when a patient is referred to One Hackney and
City as part of their discharge plan.
13. To ensure a fair process is established by which all Care Co-ordinators are
provided with the opportunity to undertake the acute sector link role in due
course;
14. To work with local providers to identify areas where there are problems and
challenges in relation to integrated service provision, and to resolve these
issues where possible. Where these difficulties remain, to ensure that the
One Hackney and City Board is kept informed about system issues which
need to be addressed. To escalate issues and risks as appropriate;
15. To be a contact point for local GPs and to establish systems and processes
that will ensure a timely and appropriate response to queries from clinicians
and other providers;
16. To act as a central point for collating information from the Care Co-ordinators
about integrated working (activity, outcomes and gaps in service provision)
and to provide regular reports to the One Hackney and City Programme
Director for the purposes of reporting to the One Hackney and City Board, on
behalf of the GP Confederation;
17. To take a lead on managing the One Hackney and City Serious Incident
Reporting system and providing regular reports to the One Hackney and City
Programme Director for the purposes of reporting to the One Hackney and
City Board;
18. To take a lead on collating the One Hackney and City Risk Register and
providing monthly updates to the One Hackney and City Programme Director
for the purposes of reporting to the One Hackney and City.
19. To support the One Hackney and City Programme Director, in liaison with
Healthwatch, in the patient evaluation of the One Hackney and City
Programme, and to support training to enable staff to understand the
perspective of patients and carers;
20. To support the One Hackney and City Programme Director in the
development of effective patient information.
Leadership
21. To provide effective and visible team leadership and day to day management
for the One Hackney and City Care Co-ordinators (5 staff) and Administrators
(3 staff), to include (but not limited to) staff development, appraisals, objective
setting, operational support, managing absence and conflict resolution;
22. To be responsible for the provision of support and guidance to the One
Hackney and City Care Co-ordinators and Administrators in delivery of their
duties;
23. To support the Administrators in developing systems to support the Care Coordination processes;
24. To provide shadowing/mentoring/coaching opportunities as identified/required
to the whole Care Co-ordination team;
25. To lead team meetings, which will include reviewing of caseload, and to
ensure that team members are supported in relation to prioritising workload
issues and making the most effective use of time and capacity;
26. To ensure that all team members undertake all statutory and mandatory
training, and are particularly aware of their responsibilities in relation to
safeguarding vulnerable adults and children, and information governance and
confidentiality;
27. To ensure that Care Co-ordination team members have access to and
participate in appropriate training and development activities to ensure that
team members develop the skills and competencies required to meet the
needs of the service.
28. To develop a process and associated support and training whereby
Administrators can be developed into Care Co-ordination roles in due course,
where they wish to undertake this role.
29. To be responsible for ensuring there are robust systems in place for managing
leave, including sickness absence;
Confidentiality and Information Governance
30. To ensure that confidentiality of information about patients, families and staff
is maintained in line with the GP Confederation’s Confidentiality policy.
31. To observe and maintain strict confidentiality with regards to any
patient/family/staff/records and information in line with the requirements of the
Data Protection Act;
32. Any data that is taken/shared as part of a phone call or transported, faxed or
transferred electronically must be undertaken with regard to the GP
Confederation’s Information Governance and Information Security policy and
procedures.
