Internship Letter of Agreement Name of Intern, Position Organization Date: February 6, 2016 This form constitutes an agreement between Your Organization and Name of Intern, Position for the services described in the included deliverables. The terms and conditions as set out herein are not subject to any other written or verbal condition, qualification, or agreement. Term: The term of the agreement shall be from Date to the date on which the agreed upon deliverables have been satisfied (within the term limits set out by the SCIP program), executed in person, via telephonic and electronic consultations and independently. Intern Goals: Name of Intern, Position will be responsible to Your Organization for goals as outlined in the Position Description. These deliverables include: Specific individual tasks to be completed Meetings to attend Additional roles to play PLEASE NOTE: your deliverables here should match is the deliverables outlined in your role description Organizational Responsibilities: Your Organization will be responsible to Name of Intern, Position for deliverables as outlined and as required to enable the project’s activities. Additionally, Your Organization will provide: A primary supervisor: Name Timely response to questions and requests and access to appropriate information; Feedback on performance and deliverables; Monthly update meetings between the primary supervisor and the intern. -1 of 3- 2009 Vantage Point Adapted for SCiP (Volunteer Alberta) June 2011 Letter of Agreement between Your Organization and Name of Intern, Name of Role Reporting: Name of Intern is accountable to Name of Person, Position. Remuneration Upon completion of the agreed deliverables, Your Organization and Name of Intern will submit the appropriate documents to Volunteer Alberta. Once Alberta Enterprise and Advanced Education has processed the applications, a bursary in the amount of $1000 will be paid directly to the student intern. Organization name can provide a letter of recommendation and/or act as a reference as appropriate. Expenses: Your Organization will pay reasonable expenses, upon submission of receipts and with the prior approval of the Name of Position. Liability/Insurance: Your Organization will be responsible for all insurance and liability coverage required for the duration of the internship. Ownership: The copyright of any reports/products developed for Your Organization relating to the deliverables during this agreement will belong to Your Organization exclusively. Confidentiality: It is expected that Name of Intern will not divulge any confidential information concerning Your Organization or its dealings with individuals or organizations. Name of Intern will review and abide by the Privacy Policy Statement and Confidentiality portions of the Intern Handbook. Termination: This agreement may be terminated with 2 weeks written notice to Volunteer Alberta, by either party. If the result of a conflict, all steps laid out in the Conflict Resolution Procedure (Appendix 2) will be followed and documented prior to termination of the contract.
