Introduction Joyce Duckworth (JD) welcomed everyone to the meeting and introductions were made. 1 Knowsley Learning Disabilities Partnership Board Health Sub Group 2. Notes and actions of the meeting held on 1 December 2011 Photosymbols Package - (V4) now available for Council and PCT use. LD Health Self Assessment Framework – on agenda for discussion Access to LD Team/Integrated Disability Team leaflet - Sarah confirmed the leaflet has been started but not yet completed. Epilepsy - To be included on the agenda for discussion at the next meeting AOB – It was noted that Whiston Hospital are still progressing well with raising the agenda on improving access for people with learning disabilities. JD confirmed that Health is now a standard item on the LDPB agenda. It was agreed to discuss with Deb Lawson the possibility of expanding the LD / ASDG Strategy Group (STIG) meeting for health input. The group agreed to continue with 3 monthly meetings and use of the North Huyton PCRC as the venue. Actions By Who? Terms of Reference Due to the strategic changes currently taking place across the Borough the Health Sub Group discussed its Terms of Reference (ToR) established approximately 18 months ago. It was agreed that a covering email will be sent out to all members with the amended draft Terms of Reference. JD highlighted the Purpose and Vision of the Health Sub Group. It was agreed communication is important in promoting the strategic health project and the need to invite health professionals and provider services to help promote services and its users. 2 Knowsley Learning Disabilities Partnership Board Health Sub Group A discussion took place as to whether there is a need to include areas of physical disabilities and mental ill health as those attending the group are learning disability professionals. It was acknowledged that the Health Sub Group is accountable to all Knowsley Partnership Boards. Due to the ongoing changes and the capacity and commitment needed the Task and Finish Working Groups have yet to be established. Therefore there is a need to re-establish a strong focus with all extra curricula groups in Knowsley. Membership The two tier membership (core and associate) was discussed. JD informed the group that Katie Hardaker, Development Worker from the Self Advocacy group BIG has now left and Paula Lomas has taken over this role. JD will contact Vicki Hornby re representation from the Autism Service Development Group and Paula Lomas regarding Self Advocacy representatives. It was agreed to delete the word ‘core’ and the associate members listed. Report back from the LD Pathway Group will be included as a standard item on future agendas. Associated members will be invited to attend when required. Chairpersons The Health Sub Group Chairperson is Johanna Lee, Clinical Lead Nurse for Learning Disabilities. Sarah Jones informed the group that as from Monday 1 October the Learning Disability Team will be transferring from KIPS to 5BP LD Business Stream. This may have an impact on Johanna’s role and her availability to attend the LD Health Sub Group. Further updates will be provided upon Johanna’s return. The importance of maintaining a strategic liaison lead for the group was also discussed. Stuart informed the group that Chris Bean, Integrated Commissioning Manager for NHS Merseyside has submitted a business case shared across the five boroughs. 3 Knowsley Learning Disabilities Partnership Board Health Sub Group Cathy Rafferty stated she is aware of 3 similar health sub groups set up across Merseyside (Liverpool, Sefton and Knowsley) although is not sure how they are operating strategically. Establishing GP representation on the Health Sub Group was also discussed and the possible difficulties around this. The group agreed SS/JD will draft an email to Tom Fairclough and Dawn Boyer to formalise an invitation to the CCG. Frequency of meetings No change (3 monthly). Where do we go from here? As the Health Sub Group has not met since November 2011 what impact has this made? Is the group making a difference? What have been the task and finish projects? Is the group just making new ones? After a detailed discussion the group identified the following areas as future agenda items: Leaflet re access to LD Team (pathway of referral) Epilepsy Working Group Dysphagia pathway Review of outputs from the Consultation of the Joint Health & Wellbeing Strategy Assessing data/information and what service users primary health care needs are 4 Knowsley Learning Disabilities Partnership Board Health Sub Group The group acknowledged the project establishing the Health Passport which has been successful across all areas including mental health and is currently working on widening this for service users with physical disabilities. Stuart highlighted the Improving Health and Lives: Learning Disabilities Observatory Health Profile for Knowsley 2012. This shows Knowsley’s outcomes are poor for emergency and planned hospital admissions. It was noted that Aintree/Whiston does have a process in place (flagging system) highlighting those with learning disabilities. Stuart also stated he has been working on Knowsley’s Health Self Assessment which is to be submitted shortly. As mentioned earlier, the biggest challenge and most important is around communication, particularly in relation to referrals. JD stated she would be happy to liaise with Johanna to establish a workplan for the Health Self Assessment. This would be supported through workstreams identified by the Health Sub Group. Establish a workplan to support the LD Health Self Joyce Duckworth / Assessment Johanna Lee Forward template / link for the LD Health Assessment Stuart Sheridan 5. 2012 LD Health Self Assessment The link below was circulated to the 2012 LD Health Self Assessment which will be submitted later today. http://www.improvinghealthandlives.org.uk/projects/self_assessment/ The template for the assessment was only released 6 weeks ago and has proved to be a difficult task due to the current changes taking place across the Borough. Stuart highlighted the various information being requested which is proving difficult to plan as the information requested changes year on year. It was also noted that feedback from the Health and Wellbeing Strategy Consultation event cannot be submitted or used as evidence as the assessment does not allow free text / comments. 5 Knowsley Learning Disabilities Partnership Board Health Sub Group Feedback from the Health Self Assessment will be available in November. Stuart also outlined the Section 75 Agreement which will now transfer to Knowsley CCG. The Local Authority is now the lead for Learning Disability Health Care. 6. AOB Mainstream funding from KCVS JD confirmed mainstream funding is available from KCVS for those who are working to promote various health initiatives for those aged 40 – 74. JD agreed to find out details re criteria and deadlines. Pathfinding at Aintree (Signage) Work is currently taking place in partnership with Mencap re hospital signage for those with learning disabilities and their carers.
