Pathophysiology and Treatment of Bipolar Disorder as assessed by In Vivo Imaging i. We are looking for adults, between the ages of 18 to 65 with a diagnosis of BIPOLAR DISORDER or MAJOR DEPRESSION and who are currently depressed, to participate in a research study conducted by Stony Brook University Hospital and Yale University. We are offering six months of free outpatient treatment and one month of free medication. We will compensate you up to $450 for your time and effort if you are eligible and choose to participate. Oxidative Stress Biomarkers in Anhedonia and Major Depression i. The purpose of this study is to look at how brain chemicals change in depression and with treatment. Anhedonia, or impair ability to experience pleasure, is a core symptom of depression, a major cause of difficulty and leading cause of lost work time for Americans, and can frequently lead to suicidal thoughts or behaviors. This study is trying to find out how brain chemicals are changed in depression and if treatments for depression can also be effective in readjusting these brain chemicals if they relieve depression. Your participation in this research study is expected to last 1-4 visit days spread over no more than 15 months. A total of 75 people will be recruited for this research study. You may qualify to take part in this research study because you suffer from depression. b. A Prospective, Double Blind, Randomized, Controlled Study to Evaluate the Safety and Efficacy of the Deep Transcranial Magnetic Stimulation (DTMS) (with the HAC Coil) for the treatment of Post-Traumatic Stress Disorder (PTSD) i. This research is being done to see if deep repetitive transcranial magnetic stimulation (DTMS) is effective in treating subjects suffering from PTSD. Deep rTMS using the HAC-Coil for treatment of Post-Traumatic Stress Disorder (PTSD) is investigational, which means that this therapy is not approved by the United States Food and Drug Administration (FDA). Repetitive transcranial magnetic stimulation (rTMS) is a brain stimulation technique. It involves generating a brief magnetic field in a coil that is placed on the scalp. The magnetic field passes through the skull and induces a weak electrical current in the brain that briefly activates neural circuits at the stimulation site. DTMS done in this study is a form of rTMS, but DTMS targets a deeper area of the brain using a specially-designed coil. ii. The Brainsway DTMS system is being tested for this purpose in this study. The DTMS system consists of the HAC-Coil, a magnetic stimulator, a cooling system and a positioning device. The DTMS system uses many weak electromagnets located around the scalp and focused at a region deep in the brain. The DTMS system targets brain regions not reached by the traditional figure-of-8 coil. The DTMS system is not approved by the Food and Drug Administration (FDA) for treatment of PTSD. Approximately, 166 men and women, aged 22 – 68 years, who have PTSD, will participate in this study. iii. If you join the study, you cannot participate in any other study, and you cannot receive additional TMS other than the TMS given in this study. c.
