Tuition-Based All-Day Kindergarten Program Financial Agreement
Please complete this form and mail it or bring it to the elementary school where your child has been placed in the Tuition-Based All-Day
Kindergarten Program. Be sure to include your payment (see options below). In order to maintain your child’s placement in the all-day
kindergarten program, the full year’s tuition payment or first monthly deposit should be made at the time of placement in the ADK
program, but no later than two weeks after the date you were notified of your child’s placement in the all-day kindergarten

Full Tuition - $2,000 per year or $200 per month for ten
months (September through June)

**Reduced Tuition - $1,000 per year or $100 per month for
ten months (September through June)
If the total full year’s tuition is paid in one payment when
enrolled in the Tuition-Based ADK program, you will receive a
5% discount. If the balance on annual tuition is Paid in Full
prior to September 14, 2015; you will receive a 5% discount.
**Families may qualify for reduced tuition by completing and
submitting a Free & Reduced Lunch application. If the application
is approved, you will be eligible for the reduced rate.

Monthly Payment Schedule: If the monthly payment option is selected, your above signature acknowledges agreement to make the monthly
payments in accordance with the attached Monthly Payment Schedule. Your signature further acknowledges your understanding that if you are late
in your payment you will receive notification that your payment is overdue and must be paid within a week of receiving the overdue notice.
If the debt is not paid, it will remain on the student’s record until it is paid. The monthly payment is due one month in advance, no later than the
15th day of each month. For example: November tuition is due no later than October 15. If the 15th falls on the weekend or a day the school is
closed, payment is due the last school business day prior to the 15th. Please see the attached Monthly Payment Schedule for specific due dates and
payment options. This document is also located on the district website (www.skitsap.wednet.edu) under Student Center, Kindergarten, All Day
Kindergarten. Students withdrawn from the ADK program prior to the 15 th of the month will be eligible for a prorated refund.
**Reduced Tuition: By requesting reduced tuition, I acknowledge that my family has been pre-qualified for Free and Reduced Lunch for the 20142015 school year (which is valid through September 30, 2015). I understand that a new Free & Reduced Lunch application for the 2015 -2016 school
year must be submitted by September 30, 2015, in order for my child to qualify to receive free or reduced lunches for the 2015-16 school year.
HOWEVER, I understand that if I prequalified for Free and Reduced Lunch for 2014-2015, the reduced tuition rate is locked in for the 2015-2016
school year. Information on how to apply for Free & Reduced Lunch can be found on the South Kitsap School District website
(www.skitsap.wednet.edu) under Departments, Food & Nutrition Services, Free and Reduced Meals, or by contacting Food & Nutrition Services at
360-443-3663.
