Thrombosis Update: A Focus on the NOACS (New Novel Oral Anticoagulant Agents) with Application to Clinical Practice As health care providers in a constantly changing arena, the safe utilization and management of high risk medications such as anticoagulants remains a priority for receiving information and updates related to clinical practice and patient care. This session will focus on the assessment of the patient receiving an Anticoagulant, highlight the indications/ contraindications/ dosage regimens/ administration guidelines for new novel oral anticoagulants, and discuss specific approaches to incorporating these agents into your practice. The participant will be able to identify key characteristics of each new agent, and strengthen their clinical decision-making abilities in providing safe care for their patients receiving anticoagulant drug therapies. Presentation by Panel of Experts: Carol Patrick is currently the Corporate Director for Advanced Practice Nurses for the NorthShore LIJ Health System, as well as founder and Director of Anticoagulation Management Services at Huntington Hospital, NY. She has been actively involved in the NY Steering Council for Anticoagulation Management, as well as Huntington’s Chairperson for C.L.O.T. the Council for Leaders on Thrombosis for the past 12 years. Her focus is on thrombosis management throughout the health care continuum, VTE Risk Stratification and Prevention, as well as strategic planning and dissemination of evidence-based best practices in management of thromboembolic disease. Carol is currently board certified as an Acute Care Nurse Practitioner, Adult Nurse Practitioner, RN First Assist, Critical Care, and Nurse Executive Advanced Administrator. She is also an AHA Basic as well as Advanced Cardiac Support Instructor, actively involved in sudden cardiac death prevention and arrhythmia management. She obtained her Master’s Degree as well as her Bachelor of Science Degree from SUNY @ StonyBrook on Long Island, NY., and has presented both locally and internationally on anticoagulation and cardiac/ critical care topics for multiple organizations and audiences. Dr. Darren M. Triller , PharmD is Senior Project Director at IPRO. Dr. Triller oversees drug quality and safety programs across New York State for Medicare. Dr. Darren Triller is a clinical pharmacist with over 25 years of pharmacy experience in academic, research, clinical and quality improvement settings. Dr. Triller is a recognized innovator who has served as the principal investigator for numerous controlled trials focusing on medication safety and improvement of the medication use system. As a Senior Director at IPRO, he has led multiple successful statewide drug safety campaigns, and has provided additional technical expertise to many other IPRO projects, such as Care Transitions, Hospice, Medication Reconciliation and Chronic Kidney Disease. He has served on multiple national panels developing quality measures for medication use, and is a presently directs statewide anticoagulant under the CMS QIO 11th Statement of Work. He services on the Medical and Scientific Advisory Board for the National Blood Clot Alliance and the Academic Advisory Committee for the Pharmacy Quality Alliance. Dr. Triller has published a variety of peer-reviewed manuscripts on drug safety topics, and has received numerous awards for his academic and clinical capabilities. Anne Myrka, R.Ph., MAT, BCPS is a Drug Safety Pharmacist at IPRO. Graduating from Albany College of Pharmacy with a Bachelors of Science in Pharmacy in 1992, Anne Myrka is a practicing licensed pharmacist in the states of New York and Vermont who is currently employed by IPRO, a quality improvement organization located in Albany and Lake Success, NY. Prior to joining IPRO she taught at Albany College of Pharmacy and Health Sciences in Albany, NY and Southern Vermont College in Bennington, VT after attaining her Masters of Arts in Teaching (science/biology) degree from the State University of New York at New Paltz in 2003. Her professional experience includes quality improvement, long term care consulting, hospital, homecare and community pharmacy practice. Ms. Myrka received certification as a geriatric pharmacist (CGP) in 2005 and achieved Board Certification in Pharmacotherapy Specialty in 2006 (BCPS).
