*Secondment opportunity to develop practice based mental health resources*
We are looking for a dynamic person experienced in primary care to lead on the implementation of the mental
health community incentive scheme (CIS) on behalf of the CCG.
The CIS seeks to provide an enhanced level of care in a primary care setting. General Practices will be incentivised to
take on additional tasks and responsibilities with respect to people being discharged from secondary care mental
health services.
The role involves working with practice staff to develop resources and capacity to enable practices to deliver the CIS.
A key objective for the post is to ensure practices are actively participating in the scheme, have organised data and
information systems and are establishing effective working relationships with delivery partners. The post holder will
work collaboratively with colleagues from all sectors including secondary care, GPs and other practice staff,
voluntary organisations, peer supporters, primary care workers etc to foster a networked approach to providing
support.

Rapid Access Proctology Clinic (Please see attached guidance)
Dear Doctor.
As you know, we run an emergency proctology clinic (previously known as the Rapid Access Proctology Clinic) on
Tuesday evenings and would welcome your patient’s referrals. We see patients with colorectal symptoms i.e. rectal
bleeding, change of bowel habit, acutely painful anal conditions and patients with anal swellings.
A patient information sheet is attached and we would be grateful if you would give this out to your patients, so that
they have the clinic information prior to attending.
We generally do not see:
a.
Patients with chronic symptoms.
b.
Elderly patients or wheel chair users who require the patient transport service.
c.
Children under the age of 16, who should be referred to a paediatrician.
d.
Patients whose examination may require general anaesthesia, such as some
disabilities.

Patients for follow up (including post-operative).
f.
Patients who are under active follow-up by a colorectal consultant.
g.
Patients who have been seen by another team with the same complaint or attended the Accident & Emergency
Department or the walk-in centre.
Please refer the above patients to the standard outpatient clinics in the daytime. Urgent cases are usually seen
within 2 weeks of receiving a fax from a GP.
To refer a patient to the evening proctology clinic, please give your patient a written referral letter containing their
name, date of birth and NHS number and ask them to bring it with them. (Please note that referrals for these clinics
are only valid for one month). Please also print out the attached patient information leaflet and give it to your
patient.
The clinic is only held on Tuesday evenings and patients must register between 4.30pm to 7pm. Please be advised
that patients who turn up before this time may be turned away and will not be given priority. The clinic
commences at 6pm when the doctors arrive, and patients are seen in turn until 9.30pm – 10.00pm so patients may
have a long wait.
Please ensure that patients are aware of the availability of standard daytime outpatient appointments which can
be booked via Choose and Book (C&B) as well as other referral routes.
The clinic is held in Clinic A, Lanesborough Outpatient Department, Lanesborough Wing, St Georges Hospital.

A Personal Health Budget
The Department of Health has announced that by 1st April 2014, anyone in receipt of NHS Continuing Healthcare
funding will have the right to request a Personal Health Budget, and by 1st October 2014, anyone in receipt of NHS
Continuing Healthcare funding, will have the right to receive a Personal Health Budget.
A Personal Health Budget is an amount of NHS money provided to the patient to meet their health and wellbeing
needs. Personal Health Budgets can be provided in one of three ways, (i) a notional budget, (ii) a third party
arrangement, (iii) a Direct Payment. The majority of people who request a Personal Health Budget are likely to wish
to receive it in the form of a Direct Payment.
Personal Health Budgets mean that people who are eligible for NHS Continuing Healthcare will have much more say
over how their health and wellbeing needs are met. If patients are already receiving direct payments through social
services, they will now be able to continue with these even if they become eligible for NHS Continuing Healthcare.

Female Genital Mutilation (Please see attached letter & flowchart)
We are writing to you to ask for your support in relation to the clear message from Government and the NHS
planning guidance about tackling FGM in the UK. It is estimated that 66,000 women resident in England and Wales in
2001 had undergone FGM and over 23,000 under the age of 15, from African communities, were at risk of, or may
have undergone FGM.1 It is our belief that we have a particular challenge and opportunity in London to make a
difference.
