Policy and Procedures
1. Absolutely NO students are to be in the school before 8:00a.m. Students who arrive at school before
8:30a.m. must report directly to the elementary gymnasium where there is supervision. This pertains
to students who ride the school bus as well as those students who ride private vehicles or who walk to
school. There is to be no loitering outside the building, in the hallways, and/or lavatories.
2. Forest City Regional School District offers a breakfast program. All students are encouraged to take
part in this nutritious program. This service begins at 8:10a.m. on a normal school day.
3. Once students have reported to their homerooms at 8:30a.m., they must remain there unless they have
permission from the teacher to leave. Students participating in the school “Breakfast Program” should
sign out from the classroom so that attendance can be taken.
4. The homeroom period begins at 8:30a.m. This time is necessary to take care of numerous homeroom
duties.
5. The official school day begins at 8:30a.m.with the Pledge of Allegiance, patriotic song, and PBIS
Pledge. No students are permitted in the hallways at this time. All activity should cease in the
classroom until opening exercises are completed. Period 1 begins at 8:43a.m.
6. Attendance and cafeteria business should be completed after morning exercises.
7. Students in Grades 5 and 6 are encouraged to have all materials when changing classes.
8. Teachers will monitor the use of the classroom telephone by students. Please check the reason for the
request and give permission only for emergency reasons.
9. No students will be allowed in the teachers’ lounge except for instructional purposes with teacher
supervision.
10. Elementary students must have gym shoes, sneakers, etc. for gym classes. It is important that
sneakers be worn when the gym classes are held indoors. This regulation is made for the safety of the
students.
11. Students are not permitted to bring large amounts of money or other valuables to school.
12. Students will not damage school property and/or equipment. Students are expected to exercise care in
handling textbooks and other school materials. Marking or in any way damaging or destroying school
property is inexcusable. Such behavior is subject to a fine (equal to the replacement cost), and
disciplinary measures.
13. Students are not permitted to bring in pets and/or animals.
14. Busing regulations require that BUS CHANGES FOR STUDENTS MUST BE SUBMITTED IN
WRITING, except in emergencies. Groups of students switching buses cannot be accommodated.
15. Children who come to school late or who are dismissed early must bring a note to school informing the
child’s teacher who will, in turn, notify the elementary office.
16. Students arriving late must report to the office.
17. Prior to the regularly scheduled dismissal time of 3:23p.m., all students walking or being picked up
will be dismissed from the elementary office.
18. Without proper identification and/or written permission, a child will not be released.
19. Any change in mode of transportation will require a written permission from a parent and/or guardian.
20. Students are NOT allowed to bring Cell Phones, Radios, IPods, Gameboys, cameras, electronic
equipment, tape recorders, toys, games, etc. to school unless so directed by the teacher for an
educational purpose. The school will not be responsible for damage or loss should any of these
items appear in the school.

21. Students are not to engage in an action that is potentially or actually harmful to the safety of others
(running in the halls, throwing objects, pushing or shoving etc.)
22. The throwing of snowballs and other objects is forbidden on school grounds.
23. Good personal hygiene is expected of all students. School attire should always be neat, clean, and
appropriate for the season of the year and the educational atmosphere. Clothing should always be in
good taste and appropriate for a classroom setting. (See Dress Code Policy)
24. No gum chewing is allowed in the elementary school.
Attendance
Parents are asked to call the school when their son/daughter is absent from school. The state law
requires a written excuse upon the return to school after being absent. These excuses for absence
should be presented to the homeroom teacher in accordance with district attendance procedures.
Students reporting late for school must report to the office or they will be considered absent for the
day. Attempts will be made to call parents of absence students.
Written excuses may be legal or illegal based on the following criteria:
LEGAL: Examples include (but not limited to): (1) illness (accompanied by a Medical excuse), (2)
death in the immediate family, (3) impassable roads, (4) pre-approved special/educational situations,
(5) Religious holidays, (6) Illness excused by school nurse, (7) Court appointments and/or
appearances, (8) Quarantine of home, (9) Out of School suspensions, or (10) Participation in a project
by a state/country wide organization (4H).
ILLEGAL: Examples include (but not limited to) missing the bus, car trouble (transportation is
provided by the district), shopping, baby-sitting, visiting relatives, hunting, fishing, skiing, personal
business, employment, working at home, remaining at home to do school work, absences not
accompanied by a Medical/Counselor excuse, cutting class/school, and arriving late/leaving early
without a legal excuse. An illegal absence may also be one where, upon a student’s arrival at school
after his/her latest absence, a note from parent or guardian or doctor is not returned to the homeroom
teacher within three days of the students return to school. If excuses are not received within the
three days of return, the unexcused absence will be permanent.
Excessive Unexcused Absences
First Notice: Three (3) unexcused absences will result in a warning/official written
notification of absences. Additionally, the student and parents/guardians may be required to
attend a Truancy Elimination Plan Meeting at the school. This official written notification
shall be mailed home.
Subsequent Notices: A total of four (4) unexcused absences may result in action through the
District Magistrate. This official written notification shall be mailed home.
Excessive Excused Absences
Pennsylvania law states that a maximum of ten (10) days of cumulative lawful absences
verified by parental notification may be permitted during a school year. All absences beyond
ten cumulative days require an excuse from a physician. If a doctor’s excuse is not presented
within three (3) days of the student’s return to return to school, the absences will be declared
unexcused. Additionally, the student and parents/guardians may be required to attend a
Truancy Elimination Plan Meeting at the school.
After ten (10) days of chronic, irregular absence, the school district may require a written
excuse from a medical doctor as verification for any additional absence due to illness.
Educational Field Trips
Parents who wish to obtain the release of their child/children from school for family trips/educational
field trips, business, or other family activities, must request PRIOR permission from the principal.
Educational field trips may not exceed five (5) school days. Prior attendance records, previous

requests, frequency of such requests, will be taken into consideration by the administration when
approving or disapproving travel requests.
Tardiness
An illegal tardy occurs when a student misses homeroom, which ends at 8:42a.m. It is the
responsibility of every parent to insure their son or daughter attends school on time.
Cafeteria Services
Breakfast
The Forest City Regional School District operates a “breakfast program.” For students arriving on
most buses, walking, or riding in with parents, the cafeteria serves breakfast until 8:30a.m., at which
time these students must report to homeroom. For students arriving late on the buses, the cafeteria will
continue serving breakfast until 8:35a.m., at which time these students must report to homeroom.
Lunch
The Forest City Regional School District operates a closed lunch program. Due to the large numbers of
students assigned to each lunch period, it is essential that each and every student assume responsibility for
leaving his/her area reasonably neat and clean. Lunch may be carried from home or purchased at school.

Snack Time
Snack time is available to students in grades PreK4 to 4. Homeroom teachers have snack time in their
classrooms. Students may purchase milk from the cafeteria to consume during snack time. Purchases
will be made from the student’s prepaid account (see Point of Sale System).
Point of Sale System
Students are required to make cafeteria purchases through an individual prepaid account. They will
not be permitted to pay for breakfast, lunch, or milk (including snack milk) in the serving line with
cash. Parents may check on cafeteria accounts through SchoolBucks.com or by calling the cafeteria
manager at 785-2412. To check accounts through SchoolBucks.com parents must register online with
their own username and password. Usernames and passwords are permanent: the same one will be
used each year throughout a child’s career at Forest City Regional School District. What this means to
students is that all students will pay for lunch in the same way – those who receive free or reduced
lunch cannot be identified. What this means to you as parents is that, at minimum, YOU NEED TO
SEND MONEY FOR ONE WEEK’S WORTH OF YOUR CHILD’S CAFETERIA
PURCHASES AT LEAST ONE WEEK AHEAD OF TIME. Many parents elect to send enough to
last a month or even a whole school year. Homeroom teachers will collect prepayments on a daily
basis. All parents are strongly encouraged to apply for free/reduced meals for your child/children,
especially if you are concerned about your ability to pay for a week of lunches at one
time. Free/reduced applications are distributed to all families by mail in the month of August; please
return them as soon as possible. If you did not receive one or need another, you may request an
application by calling (570) 785-2410.

Wellness Policy
In an effort to reduce childhood obesity, every school district participating in the National School
Lunch and School Breakfast Programs is required by law to have in place a Wellness Policy that
addresses healthy eating and physical activity. The Forest City Regional School District recognizes
that student wellness and proper nutrition are related to students’ physical well-being, growth,
development, and readiness to learn. The school is committed to providing an environment that
promotes student wellness, proper nutrition, nutrition education, and regular physical activity as part of
the total learning experience.
According to the Forest City Regional Student Wellness Policy, all foods available during the school
day shall comply with the established nutrition guidelines as listed in the Student Wellness Plan. These
foods include foods for snacks, classroom parties, holiday celebrations, and fundraisers and food from
home. During any classroom parties/activities, ALL food must be store bought with a list of
ingredients on the package due to student allergies. The cafeteria can also assist parents/guardians with
ideas for making healthy choices related to snacks and classroom parties. This information can be
obtained by calling the cafeteria office at 785-2412.
Child Study Team
The Forest City Regional Elementary School has a Child Study Team. The purpose of this team is to
help students who are “at risk” academically, behaviorally and /or socially. The team consists of
parent/guardian, student, teacher, School Counselor, remedial teacher, Principal, School Psychologist,
Special Education Supervisor and/or any other significant personnel deemed necessary by the team.

Dress Code Policy
The Forest City Regional School District has adopted the attached school Dress Code Policy for
Grades K4 through Grade 6. The dress code is mandatory and will be strictly enforced pursuant to the
attached discipline guidelines.
General Information








All students must attend school each day attired as outlined.
Tops can be embroidered/monogrammed with “Forest City Regional” or the “FCR” logo on the
left chest (samples in Principal’s office).
If embroidered, preferred color must be purple (on white or gold) or gold (on purple).
All clothing must be appropriately sized for the child, that is, they must be no more than one
regular size larger than the student actually measures. Extra wide, extra full, extra-long, baggy or
sagging pants and shorts are not acceptable. All pants and shorts must be worn waist high.
Shorts, skirts, jumpers, and skorts must be an appropriate length.
No visible emblems, tags, or logos are allowed on uniform clothing except for the District
monogramming/embroidery (as per samples).
All colors must be solid, that is, without patterns, designs, checks, etc.
Clothing may be purchased at the store/vendor of choice, as long as they conform to this Dress
Code Policy and as long as they are “Dickie” brand or “approved equal and consistent” with the

samples on display at the Elementary School. Any “equal” must be pre-approved by the building
principal.
No denim, cargo/painter pants, wind pants, or spandex pants are permitted to be worn.
Clothing may not be embellished in any way, including adding or changing buttons or pins.
Shoes with laces must be tied.
Any shoe that poses a safety hazard is not permitted; for example, flip-flops, shower shoe, and
clogs.
Socks with offensive designs or colors are not acceptable.
Hats/caps, kerchiefs, bandannas, headbands or other head coverings are not permitted while inside
the building.
Chains worn on clothing, spiked bracelets, necklaces, or dog collars are not permitted.
Make-up is not permitted.
Sweaters that conform to optional attire guidelines may be worn over knit polo or golf tops. Other
sweaters or sweatshirts may not be worn under tops while in school.

a.) Students must wear sneakers during their physical education classes. If students choose
to wear dress shoes to school on that day, they must bring sneakers to change into.

a.) T-shirts of any solid color may be worn during physical education days. However, T-shirts must be
worn under school dress code approved clothing during the school day. When students arrive to the
gym, they may remove school dress code clothing so that only the solid color T-shirt is visible.

Dress Code Discipline Policy
Any student arriving at school with clothing that does not conform to the dress code will receive a
warning and will be required to change into clothing that meets policy requirements. Upon the second
and subsequent offenses, the student will be assigned to in-school suspension. New transfer students
will be permitted 10 school days to obtain clothing to comply with this Dress Code Policy.
 In the event non-compliance continues following three in-school suspensions, the student will
then be subject to out-of-school suspensions for subsequent violations.
Drug, Tobacco and Alcohol Policy
The possession or use of tobacco, drug substances or paraphernalia, including alcohol or any chemical
or medication not listed or approved by the school health office, on school property or while involved
in a school-related activity is forbidden. In addition to school buildings, this restriction extends to
school buses, school grounds, field trips sponsored by the school, or any activity related to the school.
This would include any activity for which the school is responsible, whether on or off school grounds.
The principal has the right to issue citations with the district magistrate when a student is found
smoking, using drugs, alcohol or any other chemical inside the building or on the school campus.
Students violating this policy will be subjected to the guidelines outlined in the Forest City Regional
School District Discipline Policy.
Emergency Lockdown
With security concerns at a premium, the Forest City Regional Elementary School may practice an
emergency lockdown. This drill will help faculty, staff, and students to respond to situations in a
quick, appropriate manner to maintain their health and safety.
English Language Learners
The education of students whose dominant language is not English and/or are English language
learners is the responsibility of every school district/charter school in the Commonwealth. Title 22,
Chapter 4, Section 4.26 of the Curriculum Regulations requires that the school district/charter school
provide a program for every student who is limited English proficient (LEP) or an English Language
Learner (ELL).
The Forest City Regional School District must administer a Home Language Survey (HLS) to all
students as required by the Office for Civil Rights (OCR). The results of that survey must be retained
in the student's permanent folder. For those students whose primary language is other than English
(PHLOTE), the district must also determine the student's English language proficiency. Then, ESL
instruction at the appropriate level must be provided for the limited English proficient student with
local/state funds.
After it is determined that the student is in need of ESL instruction or bilingual education, the student's
English language and native language proficiency must be determined for appropriate instructional
placement. Students must also be assessed for achievement and for program exit.
Fire and Weather Preparedness Drills
Fire drills are conducted regularly throughout the school year. The fire alarm signal is the continuous
ringing of the fire alarm bells. Fire drill routes are posted in each room. At the direction of the
teacher, students will leave classrooms when the alarm is to be sounded and exit the building via the
routes designated for each room. All windows and doors should be closed when leaving; lights should
be left on. Students must proceed far enough away from the building so that their safety is preserved
in the event of an explosion and so that fire apparatus may function if necessary. This means that
students should evacuate to the edge of the parking lot most distant from the building. Teachers will

direct students to return to class once the clear signal is sounded. Students are fully expected to
proceed quickly, quietly and in an orderly fashion.
Weather Preparedness Drills are conducted in cooperation with the state and National Weather
Services. These drills increase students’ knowledge of what they are to do in a weather emergency.
Students are expected to fully cooperate with instructions during these drills. Students will always be
expected in a weather emergency, drill or in reality, to remain absolutely silent, as communication is
critical in one of these situations.
Grading Policy
The Forest City Regional Elementary School’s grading policy is the vehicle by which students,
parents, and teachers are made aware of the student’s progress in his/her schoolwork.
The Forest City Regional Elementary School has developed Standard Based Report Cards in grades
Pre-K4 through Third. The report card reflects the Pennsylvania State Academic Standards, which are
benchmark measures that define what students should know and be able to do at specific grade levels.
They are the basis for curriculum and instruction. Parents are periodically notified of the student’s
progress. This notification takes the form of a report card, which is distributed after every forty-five
days of school. Additionally, parents with children in grades Pre-K4 through 12 are now able to access
their child’s grades on the Forest City Regional School District Parent Portal website:

Users of the portal will have access to their child’s current information as well as view their grades;
users can also view or print draft versions of their report cards and transcripts. However, if a parent
does not have a computer or internet access, they may contact the elementary school office to make
arrangements to receive a printed progress report.
To gain access to the Portal, the user (student and/or parents/guardians) must submit their email
addresses to the elementary school office. After email addresses are added, we will send home a letter
with instructions on how to create your own account at home. Please refer to the website for more
information.
Pre-K4 and Kindergarten (5) report cards cover the developmental stages of reading, writing,
mathematics, and characteristics of the successful learner.

I – Incomplete: Students receiving an incomplete will have ten school days
after receiving report cards to complete required work.
Honor Roll
In Grade 4, in order to achieve honor roll status, a student must achieve 85% or better overall in the
following subjects –Reading, Mathematics, English, Spelling, Science/Health, and Social Studies.
In Grades 5 and 6, in order to achieve “Distinguished Honor Roll” students must attain a 93% or better
overall and “Honor Roll” students must achieve an 88% average or better overall in the following
subjects –Reading, Mathematics, English, Science/Health, and Social Studies. Spelling will also be
factored in the average, but will be assigned a quarter credit each marking period.
In addition, a grade of at least 80% or “S” is required in any other subject such as Art, Music, Health,
Guidance, Library, Physical Education and Computers. Any student receiving an “S-,” “U”, or failing
grade, in behavior will not achieve honor roll status.
Guidance Department
The mission as a school counselor is to provide a comprehensive guidance program that will assist all
students in acquiring the skills, knowledge and attributes needed to become effective students,
responsible citizens, productive workers and lifelong learners.
The Elementary School Counseling Program is an integral part of the school’s total program with a
commitment to individual uniqueness, an understanding of multicultural diversity and the maximum of
human potential. The program addresses the personal/social, educational and career needs of all
students. It is designed to enable all students regardless of individual differences, to acquire the selfknowledge, educational, occupational, and career development competencies necessary to function and
contribute in a changing society. A testing program for intelligence, achievement, and interests assist
students in their daily educational programs. Information on outside agencies, coupled with personal
counseling, provide an effective referral system to aid pupils.
The School Counselor will serve as the Coordinator of the PA Assessment. Students in Grades 3 thru
6 will be given the PA Math and Reading Assessments. Additionally, the PA Science Assessment will
be given to students in Grade 4. Additional testing will be administered at the discretion of the district.
Students, as well as parents, are encouraged to use the services of the School Counseling Department.
Students may make appointments before or after school. Parents are encouraged to call Mrs. Sharon
Paone, MS, NCC, LPC, Elementary School Counselor, at (570)785-2416 or email her at
spaone@fcrsd.org for appointments during school hours.
Harassment
Willful behavior that is specifically intended to trouble, worry, threaten, slander, defame or torment
another individual for any purpose is considered harassment. This expression of intent to injure or
harm an individual or acts intended to create a fear of pending injury or harm. It also includes ethnic
harassment, which includes the use of any derogatory word, phrase or action characterizing a given
racial or ethnic group that creates an offensive educational environment. Harassment of fellow
students or district employees is strictly prohibited and will result in disciplinary consequences.
Harassing behavior by students occurring outside of school may be considered for disciplinary action
under the harassment policy.
In particular, harassment of school employees, whether it occurs on or off school grounds, may
make students subject to further disciplinary action.
Students experiencing any type of harassment are expected to report the incidents to a teacher,
guidance counselor, or an administrator. At no time should any student engage in name-calling or
threat of physical harm (harasser or harassed).

Bullying/Cyberbullying
The Board is committed to providing a safe, positive learning environment for district students. The
Board recognizes that bullying creates an atmosphere of fear and intimidation, detracts from the safe
environment necessary for student learning, and may lead to more serious violence. Therefore, the
Board prohibits bullying by district students.
Bullying shall mean an intentional electronic, written, verbal or physical act or series of acts directed
at another student or students, which occurs in a school setting and/or outside a school setting, that is
severe, persistent or pervasive and has the effect of doing any of the following:
1. Substantial interference with a student’s education.
2. Creation of a threatening environment.
3. Substantial disruption of the orderly operation of the school.
Bullying, as defined in this policy, includes cyberbullying.
School setting means in the school, on school grounds, in school vehicles, at a designated bus stop or
at any activity sponsored, supervised or sanctioned by the school. When bullying takes place all parties
involved will be detained or suspended. Local law enforcement may be contacted, which may lead to
prosecution before a duly authorized magistrate of the Commonwealth of Pennsylvania. Habitual or
extreme cases will be referred to the Board of Education for disposition.
Fighting (Physical Assault)
The Forest City Regional School District will maintain a safe and orderly learning environment.
Fighting cannot be tolerated. Students who engage in fighting, instigate a fight, or engage in any act of
violence, while in school, on school property, or on a school bus, will face disciplinary and legal
consequences.
In addition to school consequences of suspension, or, in extreme or repetitive cases, expulsion,
fighting exposes students to prosecution under the Pennsylvania criminal code. Charges of
harassment, disorderly conduct, simple assault, aggravated assault, and/or other appropriate charges
may be filed in cases which involve any of the following conditions:
1. A weapon or other potentially dangerous instrument is used in a fight or other act of violence,
2. Bodily injury is inflicted on another person in a fight or through an act of violence, or
3. The fighting or violent behavior inflicted on another person is chronic as evidenced by a
second or subsequent offense in the same school year or by a history of inappropriate
aggressive or harassing behavior.
Students must make every effort to avoid fighting. When confronted and challenged to fight, students
should seek help from the nearest adult. If assaulted, students have the right to self-defense through
avoiding, blocking, or restraining the aggressor with reasonable physical force. Regardless of who
started the fight, students actively engaged in fighting or attempting to inflict bodily injury will
face appropriate disciplinary and legal consequences.
Sexual Harassment
All students should enjoy a school environment free from all forms of discrimination, including sexual
harassment. No student should be subjected to unsolicited and unwelcome sexual overtures or
conduct, either verbal or physical from another student or from a district employee. Sexual harassment
lowers morale, is damaging to the school environment, and is also illegal.
Sexual harassment may be in the form of student-to-student, student to employee, or employee to
student misconduct. Sexual harassment is defined as unwelcome sexual advances, requests for sexual
favors, and other verbal or physical conduct of sexual nature when:
 Submission to such conduct is made, either explicitly or implicitly, a term or condition of an
individual's employment, grades or other good standing.
 Such conduct has the purpose or effect of substantially interfering with an individual's
performance or creates an intimidating, hostile or offensive environment.

Sexual harassment can take a variety of forms ranging from subtle pressure to physical assault.
Although all facts and circumstances will be considered, some examples of sexual harassment
may include:
o Threats of sexual relations or sexual contact.
o Continuous or repeated verbal abuses of a sexual nature including graphic
commentaries on the person's body.
o Sexually degrading words to describe the person or propositions of a sexual nature.
o Sexual remarks, jokes or gestures that may embarrass or offend others.
Any student who feels he/she has been a victim of sexual harassment should immediately report the
alleged harassment to a teacher, nurse, counselor or administrator. Each complaint will be carefully
investigated by the School Principal and all findings will be documented in writing. All information
obtained will be held in strictest confidence and will be discussed only on a need-to-know basis to
investigate or prosecute the matter. No student will be subject to any form of retaliation or discipline
for pursuing a sexual harassment complaint, unless such student falsifies a charge of sexual harassment
against another student or employee. Any action taken as a result of the investigation will depend upon
the facts of each case. Sanctions may range from a warning to expulsion for students, and from a
warning to termination for employees. If a parent/student has any questions with regards to this policy
he/she should contact the School Principal.
Weapons Policy
Carrying, bringing, using, or possessing any dangerous or deadly weapon in any school building, on
school grounds, in any school vehicle or at any school sponsored activity is prohibited. The intent of
this paragraph includes transporting students to school, school sponsored activities or possessing a
weapon while walking to or from school or school sponsored activities. A weapon is an instrument
of any kind that can be used to cause harm to an individual. While not inclusive, weapons include
handguns, rifles, shotguns, spring gun, air gun, slingshot, bludgeons or clubs, brass knuckles, or
artificial knuckles, dangerous chemicals, razors or any instrument that in the judgment of the
administration could be used as a weapon or mistaken for one.
Therefore imitation weapons are included. According to Act 26, violation of this policy shall require
that the student involved be immediately suspended, law enforcement officials be notified, and that the
Board of Education consider expulsion from school. Exception to this policy can be made on a caseby-case basis due to mitigating circumstances.
Health Room Services
The Health Room of the Forest City Regional School District is staffed by a certified school nurse in
accordance with the following objectives:
1. To organize and administer programs in preventative health.
2. To respond to the outbreak of disease or general illness within the school population.
3. To provide initial emergency care and make appropriate referrals to community resources.
In accordance with the above guidelines:
All students will have a yearly health conference, which includes health history, physical data, height
and weight, and vision testing. Students from specified state mandated class levels will have a
physical, scoliosis screening, dental, hearing, and a TB test. Parents will be notified in writing or by
phone if their child has failed a screening at the time of the screening.
If a student becomes ill in school, he/she must get permission from his/her teacher to report to the
nurse. If the Nurse is not in, students are to report to the office. The school nurse has the discretion
over dismissal of student for illness during the day. If, in the nurse’s professional judgment, a student
should go home, then the nurse will make the necessary arrangements. At no time should students call
home to request someone pick them up because of illness if the school nurse has determined that they

are to remain in school. Students who leave school after making this type of arrangement will have the
absence time recorded as unexcused or illegal.
If your child has been diagnosed with a contagious illness, please notify the school nurse, your
child’s teacher, and the elementary office. We may request a medical clearance from your family
doctor for your child to be allowed to return to school.
Parents/Guardians are requested to complete a data card on the first day of school. This form should
be completed and returned immediately. This form is vital, especially in an emergency situation.
Students will be sent home only with individuals listed on this data card. Individuals will be called in
order of parent/guardian preference.
Although the district strongly recommends that medications be given at home, it realizes that, in
certain situations, this is not possible. When medication must be given during school hours, certain
criteria must be followed. All medications must be taken to the Health Room by a
parent/guardian. Students requiring medication should take their medication to the Health Room
between 8:30 and 9:00a.m. The medication must be brought to school in the original container/bottle
in which it was purchased from the drugstore. It must be properly labeled. A doctor’s order must
accompany the medication. A parent's note must also accompany the medication with the time that it
is to be dispensed. Students may be allowed to carry certain medications such as bee sting medication,
asthma inhalers, or the like. Students carrying such medication must have on file in the Health Room
a doctor's order and a note from the parent/guardian.
The Health Room functions primarily as a health-screening area, and as such will not provide DAYLONG care for students. After initial care, the student who cannot return to class will remain in the
health room while a parent/guardian arrives to transport the student home. In addition, every attempt is
made to contact the parent/guardian by phone if their child has received an injury to the head or
abdominal area, even if the injury seems mild.
Lice Policy
On the first day of each school year and the first day of school immediately following the Christmas
holidays, the school nurse will inspect all elementary students for lice. At any time during the school
year, the school nurse may inspect any student for lice as the need arises.
Upon inspection, if nits or lice are identified, the following procedures will be implemented:



The child will be removed from the classroom and sent to the health office.
The parent/guardian will be called to come and pick up the child. It will be the
responsibility of the parent/guardian to transport that child home before the end of
the school day. The child will not be allowed to ride the school bus home.
The school nurse will instruct the parent/guardian regarding the proper treatment to
eliminate the nits or lice. The instruction must include: (a) the use of specially
medicated shampoos, (b) careful laundering of bedding and clothing, (c) inspection of all
other family members, and (d) the use of a fine tooth comb, tweezers, or fingernails to
pull nits off the hair. In addition, the school nurse will provide the parent/guardian with
written information that can be used at home.
In order to be readmitted to school, the following procedures must be followed:


On the first day returning to school, a parent/guardian must accompany the child, sign in
at the elementary office and report directly to the health office. The school nurse will
immediately inspect the child for nits/lice. If such are visible, the child will be sent home
with the parent/guardian until all nits and lice are removed, (it is NOT the responsibility
of the school nurse to remove any nits or lice that is found.) The child will not be
permitted to ride the school bus until notified by the school nurse.
Parents/Guardians will be encouraged to re-examine the child’s head on the 7th, 8th, 9th,
and 10th days following the original application of medicated shampoo. A second

application of shampoo will be advised to kill any newly hatched lice if all nits were not
killed upon the first application.
 Parents/Guardians are encouraged to contact the school nurse regarding diagnosis,
treatment, and progress of any particular case.
All nits and lice must be removed before a child is re-admitted to school.

Homeroom Assignments
The Forest City Regional Elementary School wants all students to succeed. The staff provides an
excellent education for our students throughout the school year. Every consideration is provided to
meet the students’ needs. With this in mind, the professional staff at Forest City Regional Elementary
School makes every effort upon completion of a school year to group students accordingly. Many
hours are spent on this very important and essential task.
Keeping this in mind, students will be placed in the most appropriate educationally sound classrooms
as per the professional staff’s recommendation.
Homework Policy
Purpose of Policy
The purpose of this policy is to promote a district-wide, consistent approach in utilizing homework in
the instructional process. Guided by professional literature, teacher’s input, and a
teacher/administrative review committee, this policy, along with supporting guidelines, is intended to
focus on the responsibilities of the teachers, students, and parents in the meaningful, appropriate use of
homework at various levels and for various disciplines throughout the system.
Philosophy
The Forest City Regional School District strongly believes that homework is an integral part of the
educational program and should reflect student needs, capabilities and/or interests. Homework
reinforces the school curriculum, complements program objectives, and relates to Pennsylvania’s
Academic Standards. It is essential since it provides opportunities for students: to prepare for, review,
practice, apply, retain, reinforce independent work study skill; to effectively use school and
community resources; and, to develop self-discipline. There is a direct correlation between academic
success and completion of assignments.
The District defines homework as assignments to be completed at home. These assignments are an
extension of the classroom learning environment, serve as independent practice, and can include (but
are not limited to) computation and problem solving, projects, essays, researching, writing notes,
studying, reports, journal responses, lab reports, and the reading of narrative and informational text.
Although all homework is not graded for accuracy, teachers have discretion in the amount of points
that can be assigned for the completion of any homework assignment. Teachers have this same
discretion in awarding points or credit for long term projects that students are to complete at home.
Teacher will make students aware of the amount of points or credit attached to long term projects at
the time these assignments are made.

Students may make up homework they have not completed as a result of excused absences. Teachers
will use their discretion in setting a reasonable amount of time for students to complete this make-up
homework. Homework is not to be used as a punishment for student misconduct.
Homework Time Guidelines
Homework time guidelines for various grade levels include the following approximate ranges:
1. Grades K5-2 –10-20 minutes per day
2. Grades 3-4 – 45 minutes per day
3. Grade 5-6 – 70 minutes per day
NOTE: These time guidelines are averages of the maximum time students may be required to spend
on homework each night. On some nights, the time necessary for completion of homework may be
less. On other nights, students will need to spend some additional time on homework.
This would be the situation when major projects are assigned. In general, teachers make this type of
assignment to be completed over a lengthy time period. Teachers would expect that students would
complete parts of the assignment on nights when they have fewer assignments from other classes.
Another factor that might impact the amount of time students spend on homework is the opportunity
for students to complete college courses or advanced on line courses. These types of course offerings
will necessitate additional time for homework due to their very nature as advanced coursework.

1. Assign homework consistent with District policy and guidelines.

Honors Program – (Grade 6)
The program encompasses a variety of talent and accomplishments. The three students with the top
academic average for the entire sixth grade year will be honored. Each student will receive a
certificate of completion to recognize the fact that all children contributed in some way to their class as
they completed their elementary schooling.
During the program students are also honored for honor roll, perfect attendance, and the President’s
Education Awards. Students must achieve specific criteria in order to receive these awards. For honor
roll status, a student must have an 88% average for the sixth grade year. He/She must also have
maintained an 80% or “S” in the areas of art, music, health, guidance, library, physical education, and
computers. Any student receiving a “U” for behavior in any quarter will not be considered. Perfect
attendance requires that a student be in school each day without having been excused or arrived late.
The President’s Education Award requires that a student earn a cumulative average of 90% or better in
fourth, fifth and the first two quarters of sixth grade, without rounding. He/She must also achieve the
advanced/proficient level in reading or math on the PSSA in fourth and fifth grades. These are
prestigious awards that undergo rigorous standards, thus it is a great honor for a student to achieve
them.
Integrated Pest Management Policy
The Forest City Regional School District uses an Integrated Pest Management (IPM) approach for
managing insects, rodents and weeds. Our goal is to protect every student from pesticide exposure by
using an IPM approach to pest management. Our IPM approach focuses on making the school
building and grounds an unfavorable habitat for these pests by removing food and water sources and
eliminating their hiding and breeding places. We accomplish this through routine cleaning and
maintenance. We routinely monitor the school building and grounds to detect any pests that are
present. The pest monitoring team consists of our building maintenance, office and teaching staff, and
includes our students. Pest sightings are reported to our IPM coordinator who evaluates the “pest
problem” and determines the appropriate pest management techniques to address the problem. The
techniques can include increased sanitation, modifying storage practices, sealing entry points,
physically removing the pest, etc.
From time to time, it may be necessary to use pesticides registered by the Environmental Protection
Agency to manage a pest problem. A pesticide will only be used when necessary, and will not be
routinely applied. When a pesticide is necessary, the school will try to use the least toxic product that
is effective. Applications will be made only when unauthorized persons do not have access to the
area(s) being treated. Notices will be posted in these areas 72 hours prior to application and for two
days following the application.
Parents or guardians of students enrolled in the school may request prior notification of specific
pesticide applications made at the school. To receive notification, you must be placed on the school’s
notification registry. If you would like to be placed on this registry, please notify the district in
writing. Please include your email address if you would like to be notified electronically.
If a pesticide application must be made to control an emergency pest problem, notice will be provided
by telephone to any parent or guardian who has registered such notification in writing. Exemptions to
this notification include disinfectants and antimicrobial products, self-containerized baits placed in
areas not accessible to students; gel type baits placed in cracks, crevices or voids, and swimming pool
maintenance chemicals.

The Forest City Regional School District believes that the internet can allow for a creative, collaborative, and

Library
The library contains books, periodicals, newspapers, Internet-connected computers, and other sources
of reference necessary for reports and class projects, as well as for enjoyment. Library regulations are
designed to enable the library to function effectively and provide the best possible service for the
greatest number of people, and to assist students in developing good study habits for lifelong learning.
Students are responsible for all borrowed materials. Students will be charged for damaged or

No Child Left Behind
On January 8, 2002, President Bush signed into law the “No Child Left Behind Act (NCLB)”. Part of the
funding covered under this federal law is the Title I Program. Forest City Regional Elementary School
receives this funding for remedial reading classes.
Under NCLB, parents have the right to know the professional qualifications of their children’s teacher. The
following information may be requested about each of your child’s classroom teachers:

Whether Pennsylvania has licensed or qualified the teacher for the grades and subjects he/she teaches.
Whether the teacher is teaching under an emergency permit or other provisional status through which the

Forest City Regional Elementary School is continuing the Positive Behavior Intervention and Support
Program for the 2015-2016 school year. This elementary school-wide program is designed to be a
proactive approach to teach, monitor, and support appropriate school behavior for all students. It is a
focus on preventing problem behavior of all students at the school-wide classroom, non-classroom and
individual levels.

Pre-Kindergarten Program (K4)
Forest City Regional School District offers a research and literacy-based Pre-Kindergarten (K4) program
for all children within the district who are four years old as of the first day of school. We currently have
three classroom teachers with full day sessions. There are also six paraprofessionals to assist the classroom
teachers. Children are transported to school from their home or local daycare center on a separate bus with
seatbelts, and child-sized seats. All students receive breakfast, a hot lunch and snack every day.
In the Pre-Kindergarten (K4) program, children are participating in many lessons that are hands-on and
minds on. They also are given many opportunities throughout the day to engage in discovery learning
through play and learning centers. We encourage exploration and curiosity as they navigate through an
environment set up for learning and creativity. The teachers plan lessons that are developmentally
appropriate, which means they are achievable and challenging, for each student. The curriculum focuses on
the whole child and promotes each child’s physical, intellectual, emotional and social well being and
growth. The classroom environment is positive, warm, safe and enriched with literacy and print. Enrolling
your child in our Pre-Kindergarten (K4) program will be a positive and rewarding experience for your
child.

The Kindergarten teachers greet the Pre-Kindergarten (K4) on a daily basis.
Pre-Kindergarten (K4) students go through the lunch line to carry their own trays from the
beginning of the school year.
The Pre-Kindergarten (K4) students take a tour to see the Kindergarten classrooms in May. They
get to meet each teacher and engage in an activity.
A “Ready for School” packet is given to students to work on over the summer to prepare them for
Kindergarten.

Parent/Teacher Organization
The PTO is a Parent and Teacher Organization that plays an active role in our school. Many activities are
sponsored for the students (and their parents) throughout the school year. The organization’s annual
fundraisers are held in October and March. All money raised is used to support the Parent/Teacher
Organization’s various activities.
Meetings are held on the third Tuesday of each month starting at 7p.m. in the Elementary Library.
Membership is $2 per year. If school is cancelled, due to inclement weather or for other reasons, the
regularly scheduled meeting for that day is also canceled. The meeting will be held the following Tuesday
at 7p.m. No meeting is held in December. The last business meeting of the school year is held in April, at
which time, officers for the next school year are elected. A reception is held in May for installation of new
officers.

Response to Instruction and Intervention (RTII)
The Forest City Regional Elementary School has implemented a Response to Instruction and
Intervention (RTII) schedule. This includes a daily 30 minute period for grades Kindergarten through
Sixth. Enrichment and intervention activities are provided based on the individual needs of each
student.
This process is designed to address academic and behavioral difficulties prior to student failure.

School Security Policy for Visitors
The Board welcomes and encourages visits to school by parents or guardians or by other taxpayers of
the community or interested educators. To insure order in the schools, it is necessary for the Board to
establish guidelines governing such school visitations.
The School Code provides that the sole responsibility to make regular visits to the various schools of
the District and to report to the Board concerning such visits is vested in the District Superintendent.
Therefore, the District Superintendent has the authority and responsibility to implement these
guidelines.
ALL entrances to the elementary building are locked at all times. Parents will not be allowed to wait
in the foyer prior to dismissal. Parents may wait outside on the sidewalk as long as they do not block
the entry doors or the sidewalks.
Admittance into the building will be by a bell system located on the left wall of the main entrance.
Upon ringing the bell, visitors should state their name and purpose for the visit. Once you have been
admitted, please proceed to the elementary office to sign in and be issued a visitor’s badge. This
includes anyone who is entering the building to pick up a child at the Health Office.
The District Superintendent or Building Principal shall have the authority to prohibit entry of any
person to a school in this District unless or until all rules, procedures, and policies have been observed
and followed.
1. If the visitation is for a parent or teacher-initiated parent conference concerning the parent or
guardian’s child, it must be scheduled in advance with the teacher. Upon arrival at school, a

person visiting school for this purpose must proceed to the desk, sign in and receive a visitor’s
badge. Visitors will be escorted to their destination.
2. Upon arrival to the school, visitors must proceed to the appropriate office, sign in and receive
a visitor’s badge, depending on which door they are entering. Visitors will be escorted to their
destination.
3. Unauthorized visitors who are in the hallway; that is, those who have not followed the above
procedures and do not have a badge, will be immediately referred to the respective office.
4. If the visitation is for any other purpose, arrangements must be made in advance through the
office of the Building Principal.
5. No visitor may confer with a student in school, other than a student of whom he/she is the
parent or guardian, without prior permission of the Building Principal.
6. No visitor shall be allowed to photograph or videotape any person or any part of any building
or to tape record any conversation of any kind without prior approval by the Building Principal
and the District Superintendent.
Visitation of classrooms by persons other than school employees while a class is in progress invariably
leads to disruption of the learning process. Accordingly, such visitations are not encouraged. Such
visitations may be specifically approved subject to the following additional guidelines.
1. Where the classroom teacher of a student requests that a parent or guardian be allowed to
observe a class for a specified period and approval is obtained from the Building Principal,
Special Education Supervisor, or District Superintendent.
2. Where a specified period of observation by a recognized professional on behalf of the parent
or guardian is approved by the District Superintendent as a necessary part of the evaluation or
re-evaluation of an exceptional or thought to be exceptional student.
3. Parents who wish to volunteer within the school and school related activities must meet the
following requirements:

Terroristic Threats/Acts Policy
The Board of Education recognizes the danger that terroristic threats and acts by students’ presents to
the safety and welfare of district students, staff, and community. The Board acknowledges the need
for an immediate and effective response to a situation involving such a threat or act.
The threat, suggestion of threat, or use of violence to frighten people such as to bring alarm to the
student body, school faculty and staff, and/or the administration during the school day, during any
activity associated with the school, or during any business of the school district is considered a
terroristic threat. Students will face criminal charges, suspension from school, and expulsion under
Act 26, The Safe School Act.
Safe Schools Plan
With student safety as the pre-eminent concern of the district, the Forest City Regional Schools have
developed a district-wide safe school plan. This document may be viewed by members of the
community at the district office.
In the event of an emergency, please keep school telephone lines open for emergency calls. Ask
friends, parents, and neighbors NOT TO PHONE THE SCHOOL. Keep private cars out of the area
of the school. Keep streets clear for emergency vehicles-police, ambulance, fire, etc. tune in to local
radio and television stations for information on local disasters.

PENNSYLVANIASCHOOL CODE SECTION 12.2 (STUDENT RESPONSIBILITIES)
A. Student responsibilities include regular school attendance, conscientious effort in
classroom work and conformance to school rules and regulations. Most of all, students
share with the administration and faculty a responsibility to develop a climate within the
school that is conducive to wholesome learning and living.
B. No student has the right to interfere with the education of his fellow students. It is the
responsibility of each student to respect the rights of teachers, students, administrators, and
all others who are involved in the educational process.
C. Students should express their ideas and opinions in a respectful manner so as not to offend
or slander others.
D. It is the responsibility of the students to:
1. Be aware of all rules and regulations for student behavior and conduct themselves
in accordance with them.
2. Be willing to volunteer information in matters relating to the health, safety, and
welfare of the school community and the protection of school property.
3. Dress and groom themselves so as to meet fair standards of safety and health and
so as not to cause substantial disruption to the educational processes.
4. Assume that until a rule is waived, altered, or repeated it is in full effect.
5. Assist the school staff in operating a safe school for all students enrolled there
6. Be aware of, and comply with, state and local laws.
7. Exercise proper care when using public facilities and equipment.
8. Attend school daily, except when excused, and be on time at all classes and other
school functions.
9. Make all necessary arrangements for making up work when absent from school.

state and local school authorities.
11. Avoid inaccuracies in student newspapers or publications and indecent or
obscene language.
School Bus Rules and Regulations
Since the vast majority of students utilize transportation provided by the district, it is essential that
both students and parents understand what is required of those utilizing this service. When a student
rides a bus/van, misconduct of any kind is a discipline problem; her mode of district transportation will
not be tolerated and will result in disciplinary action. Depending on the nature of the infraction,
discipline could include detention, suspension, restitution, legal action, or expulsion. Please be aware
that buses have the video/audio capability to record bus conduct. For the most part, however, student
misconduct on buses is governed by the Disciplinary Code adopted by the Forest City Regional School
District. The following rules and regulations pertain to the Disciplinary Code:
1. Students should arrive at the designated bus stop 5 minutes early. Students should use care when
approaching the bus stop; remain off the roadway while awaiting the arrival of the bus and wait for
the bus to come to a complete stop before trying to board.
2. The student is only to ride the bus which is assigned. They should board and depart only at the
assigned stop. A student wishing to board or depart the bus at an alternative stop must have
written permission from a parent or guardian and verification/approval from the Transportation
Director prior to the date of the proposed change.
3. Each driver will determine appropriate seating assignments based on the pickup sequence, the age
of the student and behavioral concerns. It is the driver’s prerogative to change seating
assignments as needed during the school year. Students are not permitted to leave their assigned
seats while the bus is in motion.
4. Noise on the bus must be kept to a minimum.
5. Roughhousing and/or horseplay will not be tolerated. Students should never extend his/her head,
hands, arms, or any other parts of their body out of the bus windows or doors. Nothing should be
thrown from the bus window or doors.
6. No live animals, reptiles or insects are permitted on the bus.

7. Students can only transport personal possessions which are necessary for their educational
programs; i.e., books, gym clothing, musical instruments, etc. Whenever possible, these items
should be in an appropriate bag or case. Students are responsible for the control of all such items.
At no time should personal possessions be left in the aisle or be unattended.
8. Never experiment or tamper with the bus or any of its equipment. If a student damages any part of
the bus, intentionally or unintentionally, the student and/or the parent/guardian will be held
accountable.
9. When it is necessary to cross the road after getting off the bus or in order to board the bus, the
student should cross no less than ten (10) feet in front of the bus and be sure to look both ways to
make sure no traffic is approaching.
10. Once a student has boarded a bus, they should proceed immediately to their seat.
11. Students must not leave their assigned seat while the bus is in motion.
12. Students should assist the driver in keeping the bus reasonably neat and orderly.
13. Students will not be discharged at any point between school and the regular drop while in route to
school.
14. Students are not to use cell phones while on the bus/van.
15. All other school rules and regulations as outlined in the student handbook are in effect during the
transportation of students.

School Psychologist
The Forest City Regional School District employees a Pennsylvania Department of Education certified
School Psychologist. Responsibilities include:
 Core team member of the Child Study Team in both elementary and high school. Purpose is to
work with the team developing strategies and interventions to assist the struggling student.
 Assessment of students to determine the need for Special Education services. This includes
individually administered standardized assessments; classroom observations, teacher/parent
and student input and outside agency information.
 Consultation with educators, parents, outside agencies and the community.
 Research and planning in regards to the effectiveness of academic programs, and behavior
management system. Most importantly, contributing to planning and evaluating school wide
reforms and restructuring.

parent/guardian may request screening and evaluation at any time, whether or not their child is
enrolled in the district’s public school program. Requests for evaluation and screening are to be made
in writing to the Superintendent or Supervisor of Special Education. For further information on the
rights of parents and children, provision of services, evaluation and screening (including purpose, time,
and location), the Superintendent or any building Principal may be contacted in writing.
All information gathered about a child is subject to the confidentiality provisions contained in federal
and state law. The district has policies and procedures in effect governing the collection, maintenance,
destruction, and disclosure to third parties of this information. For information about these policies
and procedures, as well as rights of confidentiality and access to educational records, the

Photograph, Videotape, or Other Images
Sometimes photographs, videotape, or other images of students are taken during school activities by the Forest City
Regional School District or under its direction. These are then presented beyond the school classroom in various schoolsponsored media, including photographs, videotape productions, newspapers, television programs, brochures, handbooks,
programs, and Internet and Online web pages. As you know, students benefit in numerous ways by taking part in creating
media publications and by displaying their pictures. At the same time, in some cases, parents or students could feel that
portraying their image could be harmful to them.
In order to protect the level of privacy you desire for your child, please complete and sign the form below and return it to
the office. Your signature releases the district from liability. The District will not use your child’s photographs,
videotape footage, or other images until the District receives this written approval from you to do so.

Video-Audio Camera Surveillance On Buses

Parent or Guardian
The Forest City Regional School District Board of Education has authorized the use of video/audio cameras on
school district buses. These cameras can record both what is said and done on buses. The video/audio cameras
will be used to monitor student behavior to maintain order on the school buses to promote and maintain a safe
environment. Students and parents are hereby notified that the content of the tapes may be used in a student
disciplinary proceeding. The contents of the tapes are confidential student records and will be safeguarded.
Tapes will be retained only if necessary for use in a student disciplinary proceeding or other matter as
determined necessary by the administration. Parents may request to review the tapes of their child if the tapes
are used in a disciplinary proceeding involving their child.
