Provost’s Checklist for Faculty Portfolios
Department Chairs:
_____ Fully completed Portfolio per Faculty Personnel Rules (III, F, 3) referenced

3. For each year period presented in the portfolio, the following basic documentation
should be included:
a. Updated table of contents with blanks for signatures from the faculty member and
all reviewing bodies attesting to the integrity of the portfolio’s contents as listed
in the table of contents.
b. Updated vita
c. Document of Intent
d. Faculty member’s narrative
e. Performance Evaluation Form prepared by candidate, using standard
classification of performance data
f. Department evaluation
g. Reports of Decision(s)
h. Reports of Recommended Action
i. Representative evidence of teaching/job performance, research/creative activities,
and professional and public service activities. The evidence included in the
portfolio shall be representative, and thus, limited to one or two examples in each
category for each year of review. However, the faculty member should have
complete documentation available upon request.
j. Any documents produced through an appeal process or other reviews should be
included in the portfolio

Additional Points for Department Chair Consideration:
1. Consultations and reviews are two different processes. Consultations are to be completed with firstyear faculty and with tenure-track faculty in their first year of a two-year contract. Reviews are to be
completed for all other faculty.
2. The Faculty Personnel Rules require that all department review committees shall have at least three
members, one of whom is the department chair. If the department does not have sufficient tenured
faculty to meet this requirement, the constituency dean shall appoint tenured faculty of related
disciplines. See UW-Whitewater Faculty Personnel Rules III, C, 4, d for full rules.
3. When completing reviews (but NOT consultations), departments must report two decisions: 1)
whether or not to reappoint the faculty member; and 2) if the decision is to reappoint, whether to offer a
one-year or a two-year contract. Both decisions must be recorded in the department’s Report of
Decision.
4. Faculty who are hired ABD, but do not have the terminal degree completed at the beginning of the
contract year, must be converted to Academic Staff. Academic Staff review procedures should be
followed for that staff member. ** Deans Council agreed to October 1st as the final date to allow ABD
candidates to attain their PhD.
5. Faculty in their fourth and sixth year will undergo review by the Constituency Standards Committee
after being reviewed and approved by the Department and Dean. A fourth-year faculty member in the
first year of a two-year contract should consult the Department Chair for additional information
regarding the review by the Constituency Standards Committee.
