Road Map to Success
Admission to Graduate Studies
 Attend NSSLHA Brown Bags for tips on preparing for graduate study to become an SLP
 Complete Writing Proficiency Requirement prior to application, see CD website
 Attend Prospective Student Meeting, dates on CD Website
 Submit application by February 1 for fall admission
Enrolling in CD Courses
 Provide correct email and contact information to CD Clinic Office
 Register and complete CBEST exam in first semester of admission
 Attend Mandatory Student Orientation before classes begin, each semester
 Attend Group Advising every semester, or make individual appointment
 Complete Clinic request form by posted deadline
 Review CD Clinic Calendar and synch your calendar for the semester
 Join NSSLHA for current information and survival tips
 Conditional Graduate Students generally complete undergraduate course sequence prior to graduate courses
 Classified Graduate Students may be ready for first clinical practicum in first semester
ePortfolio & KASA
 Begin completing KASA (online on CD Program website) with first CD course
 Begin building ePortfolio in first semester of admission to graduate study
 ePortfolio includes graduate-level courses only
 Attend ePortfolio workshop and see CD ePortfolio template http://cddemo.efolioworld.com/contents
 Complete designated ePortfolio assignment each semester, according to CD Program ePortfolio guidelines
 Contact CD Program Faculty to schedule ePortfolio follow-up training and support, as needed
 Attend ePortfolio meetings each semester to review progress
Clinical Practicum: On-Campus
 Complete 25 Clock Hours of Clinical Observation with SLP with current ASHA CCCs
 Complete Group or Individual advising prior to registering for Clinical Practicum
 Submit Clinic Request form for upcoming semester by posted deadline in CD Clinic
 On-Campus Clinical Practicum include 2 semesters with children and 1 semester with an adult client
 On-Campus Clinical Child Practicum options include language/articulation; Autism Spectrum Disorders; AAC Child Clinic; Mission Head
Start; or SFSU Early Childhood Education Center.
 On-Campus Clinical Adult Practicum options include neurogenics; AAC Adult Clinic; and Accent Modification.
 Begin tracking clock hours with observation and clinical practicum, obtain signed clock hour forms and keep your own copies!
Child and Adult Internships: Off-Campus
 Complete Group or Individual advising prior to registering for appropriate Internship
 Submit Internship Request form for upcoming semester by posted deadline in CD Clinic
 Complete required forms for School Internship by posted deadline, see SPED Office in BH 156 (including CAP, CBEST score,
Fingerprinting, TB Test)
 Complete required steps for Medical Internship that include CPR, Hepatitis testing, TB Test, and others, as specified
 Attend required school or medical internship meetings, as posted
 Track clock hours in your internship site, obtain signed clock hour forms and, keep your own copies!
Getting Ready to Graduate
 Submit request to take MS Comps exam by posted deadline, semester before exam
 Complete and file GAP by posted deadline in the semester BEFORE graduating
 Complete and submit request to graduate in the semester of graduation
 Consult CD Program website and SFSU Graduate Studies website for specific deadlines http://www.sfsu.edu/~gradstdy
 Participate in planning CD Graduation with NSSLHA
Professional Licenses and Certification
 Attend posted graduation paperwork meetings 3x in semester of graduation
 Complete ASHA forms (KASA and application for certification) prior to final meeting in semester of graduation
 Complete CA SLP licensing paperwork (Clock hours and verification form) prior to final meeting in semester of graduation
 Submit Temporary CA license application prior to beginning work in Clinical Fellowship or Required Professional Experience site (allow
6 weeks processing time)
 Bring all completed ASHA and CA license paperwork forms for signature meeting in last week of semester
 Check with credential office in BH 244 to make sure all Speech Language Pathology Services Credential components are in place
