The Review: The student submits examples of studio work and 6 copies of a written statement concerning

Indicate areas of knowledge you feel deficient in, with respect to overall artistic goals.
After the Review: After the review is completed, each reviewer evaluates the student and writes comments
according to the criteria listed on the back of this form. After the completion of the review, the Committee will
discuss the student’s work and make a recommendation by checking A or B below:

B _______ Continue with specific requirements as listed
on the reverse side of this form.
