 Membership categories are explained on the reverse of this application form.
 A joining fee of £40 is incorporated in the annual fees shown above for all new members apart from students
and juniors. Previous adult members within the last 3 years may deduct £40.
 The playing year starts 1 April. A reduced annual fee is available to those joining after the applicable date.
 In the case of Junior members, one parent is also required to join the club as either a playing or nonplaying member, and is responsible for making the supervising adult aware of any special medical
requirements, whenever the junior takes part in activities arranged at the club.
 Please circle the fees in the table that apply to each applicant, enter the total fee in the fee due column and write
their initials alongside. Enter the total fee payable at the foot of the table.
 Your details will appear on the computer generated membership list, which is password protected.
