Evaluate care plans and outcomes including active participation in regular clinical audit. Participate in regular clinical supervision in accordance with the Trust Policy for both clinical and managerial aspects of their work. To act at all times as a professional role model for allocated learners. 3.3 Managerial To be responsible for the effective management of the defined clinical caseload and establish priorities with professional guidance from the Team Manager, Community Mental Health Team. Submit accurate information as required in keeping with the information system and additional departmental needs. To act as a supervisor in providing clinical supervision to colleagues within the Community Mental Health Team. 3.4 Personal Responsibilities To participate in the annual Performance and Development Review process. There is regular requirement for involvement in the duty practice which involves receiving referrals, screening and processing accordingly. There is a regular requirement to manage time effectively as the work pattern is often unpredictable and there may be conflict in priorities. There is frequent need to manage and assist in the prevention of risk; specifically suicide, self-harm, self-neglect, violence and aggression. A regular part of the role is to support clients in crisis and assist with crisis deescalation. Working Conditions e.g. level, duration and frequency of working in and with unpleasant, hazardous, toxic, cramped, continuous similar, awkward, extreme weather and temperature conditions, etc. The role requires frequent use of a vehicle to travel to and from appointments. There is regular need to administer depot injections and occasional exposure to unhygienic environments which may be a product of the service user’s social circumstances or mental ill health. There is occasional exposure to threats of violence and aggressive behaviour. This post is based in Primary Care settings with the majority of service delivery occurring in service users own homes.
