JOB SUMMARY:

MAIN AIMS OF POST
1.1
To maintain a clinical caseload
1.2
Assess, plan, implement and evaluate care to provide appropriate
interventions to meet the individual’s needs.
1.3
To assist in providing professional leadership as a member of the
Community Mental Health Case Supervision Team.
1.4
To maintain and enhance a close professional working relationship with
referring agencies and colleagues from other disciplines.
1.5
To be a member of the integrated Community Mental Health Team
working across the disciplines in providing a variety of services to
adults with acute and severe / enduring mental health problems.
1.6
To represent the team / service where nominated and as appropriate.

DUTIES AND RESPONSIBILITIES
3.1
Clinical/Professional

Manage a caseload utilising patient centred assessment, planning and
implementation of care plans.

Evaluate care plans and outcomes including active participation in
regular clinical audit.

Liaise with other members of the multi-disciplinary team thereby
maintaining and enhancing close professional working relationships.

Participate in regular clinical supervision in accordance with the Trust
Policy for both clinical and managerial aspects of their work.

To record and maintain all professional, clinical information of client’s
care needs in a confidential manner.

To work within the guidelines of their Professional Practice.

Carry out the nursing requirements of the Mental Health Act 1983 and
NHS & Community Care Act 1990 as required.

Adhere to local policy regarding storage, carriage and administration
(nurses only) of prescribed medication.

Keep abreast of Professional Practice clinical nursing development and
updating requirements.

To actively participate with the Primary Healthcare Team, the
Multidisciplinary Team and Care Programme Approach reviews.

To work with individuals and families at a variety of venues including
clients’ houses, hospital bases, GP surgeries etc.

Participate in the Community Mental Health Team duty service.

To act as Care Co-ordinator under Care Programme Approach where
appropriate.
3.2
Education

Supervise and assess, where qualified to do so, nursing students and
support students of other disciplines as required, providing opportunities
for learner objectives to be addressed.

To participate in educational development as required, enhancing
mental health nursing skills and knowledge with reference to learner
needs.

To act at all times as a professional role model for allocated learners.
3.3
Managerial

To be responsible for the effective management of the defined clinical
caseload and establish priorities with professional guidance from the
Team Manager, Community Mental Health Team.

Submit accurate information as required in keeping with the information
system and additional departmental needs.

To act as a supervisor in providing clinical supervision to colleagues
within the Community Mental Health Team.
3.4
Personal Responsibilities

To participate in the annual Performance and Development Review
process.

To act in accordance with Health and Safety at Work Act 1974 the Fire
Precautions Act 1971 and the COSHH.

Liaise with other members of the multi-disciplinary team thereby
maintaining and enhancing close professional working relationships.

To maintain a personal portfolio for purposes of professional
registration.

To carry out duties specific to your profession based on your
professional guidance/codes of practice.

JOB DESCRIPTION SUPPLEMENTARY INFORMATION
Physical Effort e.g. level, duration and frequency of lifting, carrying, walking, sitting,
standing, etc.
Carries out training for RESPECT and may use this in clinical role. There is a
frequent requirement to sit at a desk/workstation using computer/office equipment.
There is also a frequent requirement to drive to and from appointments.
Mental Effort e.g. level, duration and frequency of concentration, unpredictability,
deadlines, interruptions, etc.
There is a frequent requirement to undertake patient assessments for often more
than one hour and regular facilitation of multi-agency reviews and the occasional
chairing of service access meetings.
There is occasional involvement in tribunal hearings with patients and responsibility
for writing formal reports.
There is regular requirement for involvement in the duty practice which involves
receiving referrals, screening and processing accordingly.
There is a regular requirement to manage time effectively as the work pattern is often
unpredictable and there may be conflict in priorities.

Emotional Effort e.g. level, duration and frequency of distressing and emotionally
demanding situations, etc.
There is occasional requirement to manage challenging behaviour and occasionally
manage contentious issues within the client and/or family groups.
There is frequent need to manage and assist in the prevention of risk; specifically
suicide, self-harm, self-neglect, violence and aggression.
A regular part of the role is to support clients in crisis and assist with crisis deescalation.
Working Conditions e.g. level, duration and frequency of working in and with
unpleasant, hazardous, toxic, cramped, continuous similar, awkward, extreme
weather and temperature conditions, etc.
The role requires frequent use of a vehicle to travel to and from appointments. There
is regular need to administer depot injections and occasional exposure to unhygienic
environments which may be a product of the service user’s social circumstances or
mental ill health.
There is occasional exposure to threats of violence and aggressive behaviour.
This post is based in Primary Care settings with the majority of service delivery
occurring in service users own homes.
