T i t Healthy City Co-ordinator l e Healthy City Application Author Date Of Meeting September 7th 2009 Purpose For the Board to agree the content of the Healthy City Phase V Application. For Decision Progress Update For Information Summary Phase IV of the Healthy City Programme has now finished. Sheffield must now re-apply to be designated as part of Phase V. The Decision to re-apply has been agreed by the Health and Wellbeing Partnership Board, Sheffield First Board, Executive Management Team and Cabinet. In that paper, it outlined the strategy for our involvement in the next phase of WHO Healthy Cities. As Sheffield is an existing Healthy City, the on-line, fast track application has been made available to us (enclosed). All information must be in this application, no suplimentory information can be included in appendices. There is a strict 250 word limit to each section within this application. Recommendations 1 As the Steering Group for Sheffield’s Healthy City Programme, the Board is asked to agree the content of the application. b. Background Documents SFHW Healthy City Paper 2 1. Health and health equity in all local policies How will your city take forward the overarching Phase V theme of health and health equity in all local policies? Please outline no more than three actions in less than 250 words. 2. Strategic sign up Sheffield will embed Health and Health Equity in all policy by enhancing existing structures /strategy and developing new ones. Strategy The City Strategy is Sheffield First Parntership’s strategy that sets out the city’s strategic commitment to improve health for all people living and working in the city. Underpinning this is strategy is the Sheffield First Health and Wellbeing Plan. This strategy reflects both local and national priorities. Each priority within the strategy has both a Strategic and Delivery Lead from a range of partners, ensuring city wide commitment. The City Strategy is influenced by Sheffield City Council’s strategy City of Opportunity setting out the specific role local government has in delivering improvements in health. Also, the National Health Service local strategy Achieving Balanced Health setting out the specific role of NHS Sheffield. Structure Political: Cabinet Member portfolio for Healthy and Independent Living Director of Public Health: Directing health policy and strategy for both NHS Sheffield and Sheffield City Council, ensuring a co-ordained approach. Director of Health Improvement: Senior post supporting Director of Public Health with a specific role of improving health through local government. 3. Leadership Programme for Health Equipping city wide leaders with the knowledge and the skills to develop policy and strategy that maximises health gains for people living and working in Sheffield. Moving to more sustainable health interventions across the city. 4. Development of everyday Health Impact Assessment (HIA) Building from the experience from Healthy Cities Phase IV, NHS Sheffield and Sheffield City Council have committed to developing (HIA) protocol for the city that ensures a structured approach to assessing health for all policy developed. 2. Core themes of Phase V Core theme 1: Caring and supportive environments (important issues: better outcomes for all children, age-friendly cities, migrants and social inclusion, active citizenship, health and social services and health literacy) Please describe (in less than 250 words) how you intend to address the core theme of caring and supportive environments overall and which issues you intend to especially emphasize. These should be relevant to your local conditions, interests and priorities. 1. Health and Wellbeing Plan The Health and Wellbeing Plan sets out our partnership approach to ensuring the city creates caring and supportive environments. Priorities include: Safeguarding adults from abuse and neglect Supporting more people to move from supported to independent living Giving people choice and control of their support Supporting older people to lead a full and active life Enabling people with a severe mental health condition to work Enabling people with a learning disability to work Supporting Carers: We will develop a stronger city wide approach to supporting carers Sub structures supporting the Health and Wellbeing Plan include groups with a focus on: Older People Learning Disabilities Mental Health Physical Disability, Sensory and Cognitive Impairment Vulnerable Adults- homeless, rootless Carers (Subject to SFHW Board Approval) 2. Health Inequalities Audit The Inequalities Action Plan written in response to the audit will deliver improvement for Sheffield in reducing Health Inequalities. 3. Active Citizenship Consultation with communities through partners. New politically lead groups set up to support delivery of local government activity at a neighbourhood levels. Active Citizenship is enhanced in this process through Area Based Grants giving local people influence over delivery of services in their community. ‘Health’ is also included in the Community Development Plans 4. Community Development and Health Improved Health Literacy is being address through the delivery of Community Development and Health courses and structured support for local people to become Community Health Champions and deliver health and social care through volunteering. Core theme 2: Healthy living (important issues: preventing noncommunicable diseases, local health systems, tobacco-free cities, alcohol and drugs, active living, healthy food and diet, violence and injuries, healthy settings and well-being and happiness) Please describe (in less than 250 words) how you intend to address the core theme of healthy living overall and which issues you intend to especially emphasize. These should be relevant to your local conditions, interests and priorities. 1. Health and Wellbeing Plan The Health and Wellbeing Plan ensures healthier choices are made easier, by providing better access, information and support. Priorities include: Reduce inequalities in mortality between the least healthy one third of our population and our city average Reducing harm from Tobacco use: Increase the number of people who successfully stop smoking through the NHS Sheffield Stop Smoking Service Reduce the number of cheap and illicit sales of tobacco Increasing physical activity Access to leisure activities: We will develop a measure around improving access to leisure activities. 2. Obesity Strategy The city’s comprehensive obesity strategy has taken a holistic approach to reducing childhood obesity and includes the following strands of activity: Create a breastfeeding friendly city Empower parents to become positive role models for healthy living Make schools the heart of healthy communities To create living neighbourhoods by removing barriers and creating local environments that promote wellbeing Achieve behaviour change through social marketing Establish a network of Community Health Champions to support people in their communities to have healthier lives. 3. City Food Strategy Develop a comprehensive city wide food strategy to tackle access, quality and education issues. 4 4. Partnership The Health and Wellbeing partnership will support the our Environment, 0-19, Safer and Sustainable Communities and Economy Partnerships to be health promoting. Core theme 3: Healthy urban environment and design (important issues: healthy urban planning, housing and regeneration, healthy transport, climate change and public health emergencies, safety and security, exposure to noise and pollution, healthy urban design and creativity and liveability) Please describe (in less than 250 words) how you intend to address the core theme of healthy urban environment and design overall and which issues you intend to especially emphasize. These should be relevant to your local conditions, interests and priorities. 1. Health and Wellbeing Plan Building on the activity from Phase IV, Sheffield is actively engaged in improving health through urban development and design. The Health and Wellbeing Plan sets out the following priorities: Reducing deaths and injuries on our roads: Reduce the number of people killed or seriously injured in road traffic accidents Improving housing to improve health Increase visits to cultural activities 2. Climate Change The Health and Wellbeing Partnership will support the Environment Partnership in activity for prevention and adaptation to climate change. Including improving quality environments with a focus on air quality and encouraging a move to more sustainable forms of transport. Emergency Planning, including extreme weather incidents resulting from climate change continues to be a city wide priority including health partners.. 3. Urban Planning Sheffield’s Healthy Urban Planner, is building capacity within the Planning Department for improving public health through city design and Development Control. 4. Safety and Security The Health and Wellbeing Partnership will support the Safer and Sustainable Communities Partnership, to develop communities with a reduced fear of crime. 3. Capacity-building How will your city address training and capacity-building for health and health equity throughout Phase V? Please identify no more than three actions in less than 250 words. 1. Leadership Programme Sheffield is developing a city Leadership Programme for improving Health and wellbeing. This programme aims to equip our city leaders form different sectors and organisations to be able to maximise the health benefits of policy and strategy development. This programme will support the business case for moving health interventions further up stream, supporting the prevention agenda. The development of this programme will be informed by international collaboration with other Healthy Cities in the EU funded DECiPHEr project. 2. Public Health Statements Sheffield City Council has co-ordinated a new Public Health Co-ordination group with Health Champions from each service. This new group is designed to ensure health is embedded across the council. Annual Public Health Statements will identify what activity a service is delivering to improve health and what they intend to do to improve health for the next year. The development of these statements will be supported by the Health and Wellbeing Partnership 3. Healthy Sheffield Network To develop a network for people working or volunteering in Sheffield with an interest in health and to create a mechanism for people to meet and make new connections with partners across the city. To support better awareness of activity across the city that contributes to the improvement of health of Sheffield’s communities. To have tangible actions for improvement of health developed by the members of the network. 5 4. Networking What are the particular strengths or experience your city could contribute to the overall work of the WHO European Healthy Cities Network? Please identify three areas: 1. Leadership The development of the Leadership Programme, which will be influenced by working with the other cities of the DECiPHEr project will give good insight to international needs of city leaders, for their development of health issues. This comprehensive overview will support other Healthy Cities in the development of their city leaders to act as advocates for health. 2. Obesity Strategy Sheffield has been awarded £5mllion from the Department of Health for their holistic, partnership approach to the city’s obesity strategy. Through innovation of new practice and evaluation of the programme, Sheffield will be able to support other Healthy Cities with evidence of interventions for obesity 3. Informatics Sheffield has excellent data and informatics systems that highlight the state of the city’s health. Sheffield’s Basket of Indicators include social indicators of health to get a better picture of health for communities. The informatics ensures we target the right areas and can monitor changes in health more accurately. The cities knowledge and experience for collecting and using this data to inform policy development would be of value to other Healthy Cities How does your city expect to gain from the WHO European Network during Phase V? 1. Commitment to health Continuing to be part of WHO Healthy Cities reinforces our commitment to health in Sheffield 2. Evidence For Phase V, Sheffield would like to make better use of the evidence, experience and knowledge available to us form other Healthy Cities and develop an innovation programme to incorporate new ideas into policy development. 3. Comparison Sheffield would like to gain a better understanding of how it compares to the other cities in Europe, to have a better understanding of the progress we are making in improving health for our communities. 5. Monitoring and evaluation Confirm that: The city agrees to be externally evaluated by WHO YES Is your healthy city project systematically monitored or evaluated? 1. Scrutiny Processes Sheffield First Health and Wellbeing Performance Group, is responsible for monitoring outcomes and activity, ensuring positive health outcomes are being achieved by the implementation of the Health and Wellbeing Plan. Sheffield City Council Scrutiny Panel, is a political structure for scrutinising local policy. that policy development is in line with the local political agenda It ensures 2. Evaluation Projects within the Health and Wellbeing plan will be evaluated for their impact and effectiveness. Building on the evaluation of Phase IV Healthy Cities in Sheffield, Phase V will also undergo evaluation including a wide range of partners to assess the effectiveness of being a WHO Healthy City in improving the health of our communities. 6 7
