Due to the June 2004 Web site redesign, many DHS Web page addresses have changed. While Web addresses in online RA
messages may appear obsolete, the links will connect you to the content’s current location.

New Option for Benefit Recovery Help Desk Phone Number: As of 4/24/03 a new option will be added for the accounts
receivable area of Benefit Recovery. This option allows you to request a takeback of an overpayment. Outside the twin cities area
dial 1-800-657-3963 and select option 4. To reach the accounts receivable area directly dial 651-284-3432.

Ambulance Services: As of 5/1/03, MA will discontinue payment for the separate supply codes A0382-A0398 and A0422. The
final rule implementing the ambulance fee schedule includes supplies in the Medicare fee schedule rates. MA has implemented
the Medicare fee schedule.

Managed Care: Newborns whose birth mothers are currently enrolled in a managed care plan are guaranteed coverage under the
same health plan for their month of birth. All claims for the newborn should be submitted to the same health plan as the birth
mother. Eligibility in MMIS may not reflect this until updated by the financial worker.

Local Codes: As of 4/1/03, the following local DMS/Supply codes are no longer accepted. Use the national codes they are

2003 HCPCS Manual: with local codes still accepted by DHS is available at www.dhs.state.mn.us/provider/ref. Although codes
are listed in the manual, they may not be effective for coverage. Refer to bulletins and updates for current billing information. The
manual will be available for purchase at the Minnesota Bookstore. This manual is a courtesy of DHS. Future development will
depend on available staff and resources. You will be notified of added, changed or deleted codes on a timely basis.

Pediarix: Effective 1/1/03-3/31/03, Medical Assistance will pay AWP -5% for the combination vaccine for DTaP-Hep B-IPV
(Pediarix) procedure code 90723. As of 4/1/03, only administration will be paid. Pediarix became available through the MNVFC
program as of 3/24/03.

Minnesota Pregnancy Assessment Form (MPAF): Remember to send MPAFs to the following new address: Minnesota Medical
Assistance, DHS, PO Box 64893, St. Paul, MN 55164. If you are sending MPAFs in DHS pre-printed envelopes, please correct
the PO Box number and cross out the 3042 ZIP code extension. This change has been made to the online version of the MPAF
and the MPAF Technical Assistance Guide at http://edocs.dhs.state.mn.us (paper MPAFs are no longer available from DHS
Forms Supply).

Billing Procedure Codes X5362 and X5450: X5362 (Assisted Living Plus) and X5450 (Foster Care one month) must be billed as
1 unit per month and not according to the number of days in the month or number of units on the service agreement line. A recent
change was made in how these services are entered on service agreements but not in our billing procedures.

New Provider Updates: www.dhs.state.mn.us/provider/upd. Note that DHS no longer mails updates unless they include
legislative changes or pertain to HIPAA. All updates are posted to the Provider web site.
<br>
Hospice: Revenue billing code 659 (hospice room and board changes) changed to 658 (hospice room and board-NF), effective
for services on or after 4/1/03.

EVS & POS: will be unavailable Monday, 5/5/03, from 2 a.m. to 5 a.m.
