HISD Private Schools ESEA Title IIA 2010-11 Process and Procedures for Professional Development Services University Courses, Certification or Degree Programs: Funding-ESEA Title IIA To apply for approved certification or degree programs with partner universities: Grand Canyon University, Jones International University, National University and Western Governors’ University 1. Review program options with Mark Hatch at (866) 214-8614. 2. Teachers must complete an Intent to Apply form with principal’s signed approval. Fax the Intent to Apply form to Mark Hatch at (888) 661-2444. Administrators must obtain signed approval from consortia officials. 3. Upon approval and verification of available funds, Mark Hatch will contact each approved applicant to complete university application and required Mind Streams documents (Memorandum of Understanding, Credit Card Authorization, W-9, Approved List of Courses). 4. ESEA Title IIA funds will be used to pay tuition directly to the university. All other associated costs (computer, books, library fees, technology fees, parking fees, etc.) are the responsibility of the student. 5. Applicants must be employed at a participating non-public school with HISD boundaries during the term of the course. University of Notre Dame and University of St. Thomas Programs of Study (Catholic School Educators Only): Funding-ESEA Title IIA 1. Contact Mark Hatch at (866) 214-8614 for program descriptions and requirements. 2. Applicants must complete an Intent to Apply form, obtain principal’s signed approval and fax to Mark Hatch at (888) 661-2444. All interested administrators must obtain signed approval from Sr. Kevina Keating, Superintendent of Archdiocese of Galveston-Houston. 3. Upon approval and available funds, Mark Hatch will contact each approved applicant to complete university application and required Mind Streams documents (Memorandum of Understanding, Credit Card Authorization, W-9). 4. ESEA Title IIA funds will be used to pay tuition fees directly to the university. All other associated costs (computer, books, library fees, technology fees, parking fees, etc.) are the responsibility of the student. 5. Applicants must be employed at a participating Catholic non-public school with HISD boundaries during the term of the course. University Programs of Study-Non-Affiliated Universities: Funding: ESEA Title IIA Tuition Support for Courses at Non-Affiliated Universities (such as University of Houston, Rice University, Concordia University) 1. Applicant must complete the Request for Non-Affiliated University Course Tuition Support form prior to registering for a course. Participant must obtain principal’s signed approval. 2. Submit form with university course syllabus and summary (describing the content/outcomes) Vanessa Young Fax to (877) 505-9717. 3. Approval must be received prior to the start of class for tuition support. Mind Streams will provide explicit instructions for tuition support upon the candidate’s approval for the program. 4. Applicants must be employed at a participating non-public school with the HISD during the term of the course. Request for Reimbursement for Approved Courses at Non-Affiliated Universities: 1. Complete the Request for Non-Partner University Course Reimbursement form after successful completion of courses. 2. Submit form along with proof of payment (paid university receipt) and proof of successful completion (transcript or grade report) and fax to Vanessa Young (877) 505-9717. 3. Other costs such as registration fees, library fees, technology fees, books, parking, etc. are not eligible for reimbursement.
