UNIVERSITY OF CONNECTICUT College of Agriculture and Natural Resources DEPARTMENT OF ALLIED HEALTH SCIENCES UNDERGRADUATE PROFESSIONAL PROGRAM APPLICATION INSTRUCTIONS 1. WHEN TO APPLY: Students are required to submit a Department of Allied Health Sciences “Application for Admission to Undergraduate Professional Programs” by February 1, after completion of at least three (3) semesters of study including the required prerequisites for the program of choice. Each year, review of applications will begin on February 1 and continue until the class is filled. Allied Health Sciences applications become the confidential property of the department and cannot be returned. The Department of Allied Health Sciences WILL NOT provide copies of the application or references. Retain copies for your own files. 2. HOW TO APPLY: A. APPLICATION: Applicants must complete the Department of Allied Health Sciences’ “Application for Admission to Undergraduate Professional Programs.” Applications will NOT be reviewed unless ALL information is complete. B. COLLEGE TRANSCRIPTS: Applicants must submit documentary evidence of all collegiate work completed through the Fall Semester (including if possible spring course registration). Unofficial UConn transcript: If you have completed course work at UConn as a matriculated or non-degree student, you MUST submit a UConn transcript. An unofficial UConn transcript can be obtained by accessing your Peoplesoft account. INDIVIDUAL SEMESTER GRADE SHEETS ARE NOT ACCEPTABLE. Official transcripts: Applicants (regardless of application status) who completed course work at institutions other than the University of Connecticut MUST submit official transcripts from each school attended so that we have a grade for those courses. Please DO NOT request copies of transcripts from the University Admissions Office; as they are their “working copies”. Copies of transcripts are not acceptable to the Department of Allied Health Sciences. Applications will not be considered without such documentation. C. RECOMMENDATIONS (form required): Applicants to the Professional programs in the Department of Allied Health Sciences must supply the admissions committee with at least two (2) but no more than three (3) letters of recommendation on the application recommendation form. One recommendation MUST be an academic reference (faculty/instructor/teaching assistant). The second may be employment, volunteer or academic, but NOT personal. They should be sealed in an envelope and signed across the seal by the evaluator. It is preferable, when possible, to include sealed letters of recommendation with your application. If this is not possible, they should be sent to the address on page 5. Applicants to multiple programs need only supply two (2) recommendations. D. PERSONAL STATEMENT: Applicants are required to submit a typed 500 word personal statement outlining their career goals as they relate to the program of application. Applicants applying to more than one program are required to submit a personal statement for each program they seek admission to. E. There is NO application fee for the Department of Allied Health Sciences’ undergraduate application. 1 3. WHERE TO APPLY: SUBMISSION: Applications must be submitted no later than February 1. All supporting materials (transcripts, recommendations. personal statement, etc.) must be received in our office no later than February 1. Failure to successfully complete prerequisites, contingency requirements, or failure to maintain good academic standing may be cause for revocation of offer of admission. Additionally, applicants must also meet university admission requirements and be accepted by the University. UNDERGRADUATE STUDENTS (Storrs and Regional Campus) at the University of Connecticut: If you are a current undergraduate matriculated student at UConn, whether at Storrs or any of the regional campuses, you need only file the Department of Allied Health Sciences application to be reviewed for admission. No additional application is required for admission. However, if you are a matriculated student at a REGIONAL CAMPUS and you are offered admission to a program, you must (following acceptance of our offer in writing) submit a Campus Change Request Form to the Registrar’s Office at your current campus. Check with the Assistant to the Dean’s Office at your Regional Campus for application deadline details. Delay in submission of this form may impact housing if requested. READMITTED STUDENTS: If you have ever attended the University of Connecticut as an undergraduate matriculated student (whether or not you received an undergraduate degree), you are classified as a READMITTED STUDENT. Students seeking readmission must first be admissible to the university before an offer of admission can be extended by the Department of Allied Health Sciences. Therefore, you must file an application for readmission with the Dean of Students Office. While the readmission deadline for Fall is July 1, readmission applications must be on file in the Dean of Students Office by February 1 to be considered for admission to a professional program. While the university admission deadline for transfer students for Fall is April 1, university applications must be on file in the Transfer Admissions Office by February 1 to be considered for admission to a professional program.
