APPLICATION FOR POST 16 – ENTRY 2015
Dear Year 11 student.
This is the application form for entry to Coombe Dean 6th form in September 2015. If you have any specific questions at this stage,
please direct these to Mr Koehler-Lewis. Please bear in mind, you are indicating the subjects you would like to take and you will
discuss these and any clashes and changes at interview. Interviews will take place during careers lessons from February onwards
(or if you are an external applicant, by arrangement). Please complete the form in as much detail as possible, and return it to Mrs
Long in the post-16 reception area or to Mr Koehler-Lewis, ideally by Friday 9th January, but by Friday 16th January at the very
latest. The form can also be completed electronically (available from the school website) and emailed to
along@coombedean.plymouth.sch.uk.

Our vocational pathway offers the opportunity to remain a Coombe Dean student whilst still accessing a range of courses from other providers.

opportunity to choose which college/training provider you attend, provided they offer your course choice.
