CHILDREN’S CORNER COOPERATIVE NURSERY SCHOOL PARENT-SCHOOL AGREEMENT 2012-2013 We wish to enroll our child at CCCNS for the 2012-2013 school year. Classes will be held beginning the week of August 19, 2013 through May 22, 2014 with orientation during the week before school starts. We agree to pay tuition payable as described in TUITION below. We fully understand and agree to the following conditions of membership (a full and detailed list of CCCNS policies can be provided upon request to either a Board Member or teacher): TUITION Tuition is paid in nine (9) installments with the first installment due by June 1 st . Tuition is established by the CCCNS Board and is based on the number of attendance days per week the parent member has selected for their child. Full tuition is due the first (1st) day of each month beginning September thru April, regardless of absences or school closings. Tuition paid after the fifth (5th) day of the month may be subject to a fine. Checks are made payable to CCCNS and can be mailed to P.O. Box or delivered personally to CCCNS. Refunds or deductions shall not be made for absences or school closings. Tuition is payable until the end of the school year unless the child has been withdrawn prior to March 15. If the withdrawal is prior to March 15, the balance of any tuition paid in advance will be refunded. Any tuition still outstanding will be assessed. If no written notice is provided, tuition will be charged for two weeks after the last attendance date and the cost of a paid substitute will be billed to the parent. TRANSFER This agreement shall be null and void if the child transfers to another program at CCCNS, and a new parent-school agreement shall be signed. PARTICIPATION Participation of membership has two areas of obligation. First is direct participation in the classroom and second is direct participation in the co-op effort. Participators will receive instructions from the teacher regarding their duties If for any reason the parent member is unable to participate as scheduled, a trade can be made with another parent or a substitute can be arranged prior to the scheduled day. A paid substitute selected from a list of approved substitutes (arranged for and paid by the parent) is allowed once per semester per member when the child attends less than five days per week. The parent of a child attending full time (five days per week) is allowed two (2) paid substitutes per semester. A “no-show” for a scheduled classroom participation day results in a $30.00 fine and a scheduled makeup date. Two “no shows” for classroom participation days result in a $37.00 fine plus a make up date. Subsequent “no shows” could result in termination of membership. Parent members are also expected to assist in various jobs as necessary to benefit, beautify, repair, work in end-of year cleanup, fund-raisers or other endeavors sanctioned by the Board and the Parent Coordinator. In addition CCCNS has a variety of committees that function for the advancement of the school’s welfare. ADJUSTMENT PERIOD The first 4-6 weeks shall be regarded as an adjustment period for both parent and child. We prefer that all children with the exception of those enrolled in the 1 day class are fully potty trained. However, we will permit children in the process of potty training to attend school wearing pull-ups. For a child having difficulty adjusting to school, the teacher may suggest a modified program. The Executive Committee, in consultation with the teaching staff, may request withdrawal in cases of poor adjustment of a child or unsatisfactory participation of a parent. HEALTH Prior to the first day of school parent members are required to provide a written certificate from a Doctor of Medicine stating the child is physically able to participate in the CCCNS program and proof of immunizations. Allowances may be made in the event of religious conflict, provided no other participating child is at risk. It is the expectation of CCCNS that children will be in attendance only when in good health and capable of full participation (including playground) in school activities. Indicators that a child should remain at home include: recovery from physical injury, fever, cough, rash, eye inflammation, ear drainage, vomiting, diarrhea, sore throat, open sores or any condition that has the potential of being communicable. In the event a child becomes ill at school the teacher will contact a parent to pick up their child. Should an unforeseen emergency occur the teacher will follow instructions as stated on the “Emergency Release Form.” In potential life threatening situations the teacher will take immediate actions that serve the best health interest of the child and a parent will be notified. SCHOOL RESPONSIBILITY The school shall be responsible for each child from the time he/she is put in the care of the teacher or receiving parent at the beginning of the school session until he/she is turned over to the person who is to take him/her home at the end of the session. Each child is covered by accident insurance, which is paid by the school. We agree that our child may take part in all school activities unless we specify otherwise. PARENT LIABILITY Children’s Corner is first and foremost a cooperative effort - a preschool collectively owned by its children’s parents and operated for our children’s and our own mutual benefit. Your considered commitment to all the terms of this agreement helps ensure that it is a successful enterprise. Noncompliance with these terms may result in action by the Board.
