SCHOLARSHIP REQUIREMENTS: Please submit the following required documents: 2014-2015 Scholarship Application (see page 2 of this packet) Personal Statement - One (1) page, 12 pt. font o Discuss when and why you choose Kinesiology major, educational and career goals and demonstrate your financial need. Résumé (including name address, dates of employment, job history, volunteer experience, professional and co-curricular involvement, awards and honors, and any part-time or summer employment). Two (2) letter of recommendation (from a faculty, staff or employer/supervisor). A copy of your unofficial transcript (available on MyCSULB) The Kinesiology Department offers multiple scholarship opportunities for students: Please check the box of the scholarship(s) to which you are applying. Dorothy Deatherage Scholarship ($500) Must be a graduate student in the Kinesiology Department in the final year of study who has completed an Advancement to Candidacy Must have at least a cumulative 3.5 GPA Must demonstrate outstanding achievements during graduate studies Dorothy Doyle Scholarship ($500) Must be continually enrolled at CSULB for the past 2-4 academic semesters. Must be a full-time Kinesiology student in the K-12 P.E.T. option or Adapted PE options who is not graduating in the spring semester during the application is submitted. For the 2015 application cycle, a student in the K-12 P.E.T. option will be selected. Must demonstrate financial need. Kinesiology Beach Scholarship (3-5 @ $500) Must be a junior, senior or graduate student in the Kinesiology Department at CSULB Must have at least a cumulative 3.3 GPA and a 3.5 Kinesiology GPA Must be a full-time students enrolled in at least 12 units Must have completed at least 24-36 units at CSULB at the time of applications Must have been continually enrolled at CSULB for the past 2-4 semesters Kinesiology Department Professor Emeritus Scholarship ($500) Must be a junior or senior in the Kinesiology Department at CSULB Must have at least accumulative 2.75 GPA Must have completed at least 24 units at CSULB Must be an “emerging professional” student who has been involved in many different professional accomplishments beyond course requirements Ruth Lindsey Scholarship ($1,000) Must be a Kinesiology graduate student in final year of study who has completed or is in the last stage of completing a thesis or project that contributes significantly to the body of knowledge and/or profession Must plan to be enrolled in an advanced educational program no later than the academic year immediately following completion of the Master’s degree in Kinesiology Must demonstrate evidence of intellectual initiative, originality and creative activity or potential Must have contributed in a significant way to the Department and/or the profession PLEASE NOTE THAT LATE OR INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED.
