Weber State University Department of Health Promotion and Human Performance Athletic Training Education Program Master of Science in Athletic Training Application Admissions Instruction Sheet STEP 1: Weber State University Admissions Application a. Students not currently enrolled at Weber State University are required to complete a Weber State University admissions application which is found at http://weber.edu/admissions/studentapplication.html. After reading the admissions/scholarship online application instructions click on APPLY NOW and continue following the directions. This can be completed once accepted into the athletic training program. b. International students must download their application from the WSU Admissions website. For additional help either call 801-626-6005 or send an e-mail to admissions@weber.edu. c. Submit official college transcripts. See Step 3 below. STEP 2: Master of Science in Athletic Training Admissions Application a. Please download a copy of the application from Athletic Training website. If you would prefer a printed copy, please contact the MSAT Program Director at 801-626-7656 or ValerieHerzog@weber.edu. b. Type or clearly print all information unless otherwise indicated. Be sure to place your name on every sheet of the application. Submit the following: i. Completed application - please type ii. Two (2) letters of recommendation using the standardized form from the website. One must be from a college/university instructor. The envelope MUST be sealed with the signature of the individual across the seal on the back of the envelope. iii. Current copies of your Emergency Response and CPR/AED for Professional Rescuer certification cards (front and back). If you do not possess these certifications, or if they have lapsed, then you may complete them during your first semester. iv. Copy of GRE scores. If you are scheduled to take the exam or waiting for your scores indicate this on the application. Scores should be sent directly to the MSAT Admissions Committee. Only required if GPA is below a 3.0. Students below a 3.0 GPA should have a minimum score of 1000 on the GRE (verbal and quantitative combined) or other substantial evidence of academic achievement. v. Copy of the signed Technical Standards form located at http://weber.edu/athletictraining. vi. Physical examination and proof of immunizations, including Hepatitis-B Vaccination. If just stating the Hepatitis-B series, please indicate this. See the Immunization form for the list of required immunizations located at http://weber.edu/athletictraining. vii. Submit official college transcripts to the MSAT Program Director. See Step 3 below. viii. Application fee in the form of a check or money order in the amount of $30.00, payable to Weber State University. STEP 3: College Transcripts a. Official transcripts must be submitted with BOTH of your applications. One official copy should be submitted with your Weber State University application and a second official copy should be submitted with your Master of Science in Athletic Training Application. b. Students currently enrolled at Weber State University do not need to submit official transcripts. A copy of your online transcript will suffice. If you have attended any other schools you will need to obtain official transcripts for each of these schools. You may request that official transcripts be sent directly to the MSAT Admissions Committee or to you. You may then forward your official transcripts with your application, but they must be sent UNOPENED in their original, sealed envelope. c. Students NOT currently enrolled at Weber State University will need to submit official transcripts of your academic record from all previous colleges or universities attended should be submitted with your application. You may request that official transcripts be sent directly to the MSAT Admissions Committee or to you. You may then forward your official transcripts with your application, but they must be sent UNOPENED in their original, sealed envelope. d. For prerequisite courses completed at universities other than Weber State University, you will need to include a copy of the detailed course description from the college catalog or a course syllabus for each course. This information needs to be sent to ONLY the MSAT Admissions Committee. Additional Admission Requirements for International Students: 1. All international students and any applicant educated outside the U.S. must demonstrate proficiency in English. Those whose native language is not English, or whose language of instruction for their undergraduate degree was not English, will be required to submit a score from the Test of English as a Foreign Language (TOEFL) which is not more than two years old and on which a minimum score of 550 (paper-based), 213 (computerbased), or 79 (internet-based) has been earned. These scores need to be sent ONLY to the WSU Admissions Office. 2. All international transcripts must also be evaluated using one of the WSU approved Foreign Credential Evaluation Services Agencies listed on the WSU Admissions website: http://weber.edu/Admissions/is_transcripteval.html Please send copies of this evaluation to BOTH the WSU Admissions Office and to the MSAT Admissions Committee. Deadlines 1. Application deadline for early admissions is February 1st for the following fall semester. Applications received after the deadline will be considered on a rolling admissions basis if available slots still exist. Students are encouraged to apply for Early Admissions as the program may reach capacity at that point. 2. Interviews (in person or phone interviews) will be scheduled by Feb. 20th. 3. Final decisions will be sent by March 15th. Scholarships for Non-Resident Students 1. Non-resident students (including international students) who apply by the Dec. 15th deadline and receive one of the top three overall admissions scores (based on the numeric selection process), will be awarded a tuition waiver for the difference between resident and non-resident graduate tuition. Post-Admission Requirements 1. After formal admission to the Athletic Training Masters degree program, students are required to complete an FBI background check and drug test. 2. Both the background check and drug test will be completed on campus during the student's first semester. The expenses (approximately $75 total) will be paid for by the student.
