We will issue a credit card-style pass to students attending years 7 to 11. Therefore, please
send us a recent passport-style photo of the student, if attending these years. Please write
the applicant's name on the back of the photo – do not staple the photo to this form.
When sending us this form, please also include a copy of your latest benefit document,
which must be no more than 3 months old.
Declaration
I declare that:
a. the distance from my home address to the school to be attended, using the nearest available
walking route, is:





2 miles or more for infant aged pupils (ages 5 to 7)
2 miles or more for junior aged pupils (ages 7 to 10)
3 miles to 6 miles for secondary aged pupils (ages 11 to 16)
2 miles to 6 miles for secondary aged pupils (ages 11 to 16) whose parents receive benefits
2 miles to 15 miles for secondary aged pupils (ages 11 to 16) attending a faith school
b. should the child leave school or move address, I will notify the Education Awards and Benefits
service and immediately return any tickets issued to me
c. I accept responsibility for my child while they travel to and from school on the transport
provided
d. I understand any instances of bad behaviour may result in transport facilities being withdrawn
for a set period or permanently
e. should I return to employment and stop receiving benefits I will notify the Education Awards
and Benefits service immediately
f. I understand the council will cancel free transport with 6-weeks notices if the transport was
awarded on an 'unsafe route' criteria and the route is re-classified as safe for walking
Behavioural agreement
I acknowledge that should my child's behaviour be deemed inappropriate and a risk to health and
safety within the vehicle, transport may be suspended or withdrawn.
