1. Job Summary
To provide a high quality clinical risk management service to the trust. This
will involve providing advice, guidance and practical activities such as risk
assessment and incident investigations across all areas of trust activity. To
assist and deputise for the AD/N Clinical Governance and Risk Management
in the implementation of clinical risk management strategies and projects
throughout the trust.
The post holder will actively promote collaboration across directorate and
professional boundaries with the aim of developing and embedding a sound
open and fair culture within the trust. A key activity to promote this culture will
be through the delivery of clinical risk management training courses to all
levels of trust staff.
2. Clinical Risk Management
2.1
Review all clinical incident forms and ensure they are appropriately
investigated and ensure actions are taken
2.2
Analyse and interpret incident data identifying trends. Lead on trust
initiatives to reduce clinical risks identified by incidents work with
relevant staff and near misses and work with trust staff. Make and
present recommendations to improve patient safety within the trust
and ensure that these are followed through

Advise and support the clinical risk directorates on clinical risk
management development / implementation at directorate level
2.4
Develop tools for risk indicators (eg pressure ulcers and infection
control) and ensure implementation of action plans
2.5
Contribute to the trust’s risk register
2.6
Maintain a positive relationship with clinical staff to ensure effective
communication of all issues relating to clinical risk
2.7
Ensure the risk management database reporting function is timely and
summarised reports are produced monthly for the clinical directorates
and bi-monthly for the Risk Management Committee
2.8
Comply with the trust’s serious untoward incident policy (SUI) and
advise on its use. Assist in implementing and monitor the application
of any recommendations arising from SUI investigations. Report
progress at directorate level and feedback to the appropriate outside
agencies as necessary
2.9
Assist with any serious untoward incident investigations as per trust
policy, providing specialist advice to trust staff
2.10
In partnership with relevant staff to carry out root cause analysis, (in
line with guidance and training from the National Patient Safety
Association,) meetings following serious incidents
2.11
Oversee the administration and functioning of the trust’s Clinical Risk
Committee including the production of reports for the Clinical
Governance Steering Group and other trust committees as required
2.12
Deputise for the AD/N Clinical Governance and Risk as required on
key risk and other Trust committees and outside meetings as required
2.13
Edit the bi-monthly Clinical Risk Newsletter “Nine Lives” with the help
of the AD/N Clinical Governance and Risk. Contribute with summary
articles.
Training and Development
2.14
Develop and deliver a written clinical risk-training plan in collaboration
with the education and training team and clinical audit
2.15
Respond to requests from the directorates for risk training and
education sessions
2.16
Deliver the clinical risk training modules in accordance with the trust’s
education and training programme
2.17
Contribute to the trust’s staff professional development and education
programmes.

Be responsible for overseeing the preparation of CNST evidence and
management of the Level 2 assessment process
2.19
Deliver any teaching and training required for the implementation of the
CNST standards trust-wide.
3. Management Responsibilities
3.1 Line-manage the clinical risk co-ordinator and the clinical risk
administrator and ensure objectives are delivered
3.2 Carry out appraisal and performance management if required in line with
Trust policy
3.3 Provide support guidance and counselling for risk management staff.
Monitor and manage their sickness, absence and annual leave.
