Driving/Carpool Permission and Release
My child, ___________________________, has permission to drive/ride to the Medina
County Career Center on each of the Junior Leadership Medina County program days as
listed on this year’s program schedule.
I understand that program days begin at the Medina County Career Center at 8am and
conclude at the same location at 2pm. Opening and closing retreats are on Saturdays (as
well as Friday evening, Sept. 27) as listed on the 2013 -14 program schedule.
Transportation to the morning meeting site (Career Center) is the responsibility of the
student. Transportation will be provided during the program days, between sites, by
various school buses, owned and operated by public schools in Medina County.
My child will be riding with/driving with the following students:

I have taken the steps to ensure the appropriate, safe, and legal travel of my child
to and from each program day and understand that Junior Leadership Medina
County and Leadership Medina County is not responsible for the same.
I hereby release Junior Leadership Medina County and Leadership Medina
County, from all claims, demands, rights, causes of action, losses, injuries,
damages, liability, costs, expenses, interest, and fees (including attorney’s fees) of
every nature and description whatsoever, in law or in equity, arising in connection
with my child’s travel and transportation to and from each Junior Leadership
Medina County program day.
All students listed must submit a signed permission and release form.
