FORMAT OF CHAMPIONSHIP
18-Hole Selected Drive Alternate Shot (Teams will select the best tee shot on each hole and then they will play alternately for the remainder of the hole). Teams will consist of a HGA Junior Golfer along with their parent/guardian. Parents must submit a USGA Handicap Index
(see entry form). All players may use a golf cart in this competition.
ELIGIBILITY
Entries are open to all current 2007 HGA Junior Golf members who are in the Boys 16-18, Boys 14-15, Boys 12-13, Girls 12-14 and
Girls 15-18 age group along with their parent/guardian. Flights will be determined by the junior participant’s age as of August 7, 2007.

Complete and return this application to the HGA, 5810 Wilson Road Suite 112, Attn: Junior Golf Office, Humble, Texas 77396. Please enclose your
$200 entry form and check made payable to the Houston Golf Association. Tournament applications and the proper entry fees must reach the
Houston Golf Association by 5 p.m. on Monday, August 20, 2007. Late applications will not be considered for selection. If you have any questions please call (281) 454-7000. If your check is returned for insufficient funds your entry will be removed from consideration, even if it had been accepted prior to the check being returned.
CONDUCT CODE
Any abusive language, club throwing, cheating, disrespect of volunteers or officials, abuse of the course, leaving the course during your competitive round without permission of an HGA Official, any use of drugs or alcohol and any vandalism at a tournament venue will be considered violations of the Conduct Code. The penalties for Conduct Code violations, based on the severity of misconduct, may be any of the following: a warning, disqualification of the player from the tournament, suspension of the player indefinitely from all HGA events.

I, for myself and the player, hereby release the host facility and the Houston Golf Association, its officer, directors, employees and volunteers from any and all liability for any event or consequence whatsoever in any way arising out of or relation to members participation in an HGA event with the sole and singular exception of liability arising out of bad faith or willful misconduct.
