The normative data files, scoring algorithms, results, documentation, SPSS syntax files and questionnaires (hereinafter referred to as the AAOS Outcomes Material) contained in the outcomes section of the American Academy of Orthopaedic Surgeons web site are protected by copyright laws and international copyright treaties, as well as other intellectual property laws and treaties. The American Academy of Orthopaedic Surgeons and others collectively own all title and copyrights in or have permission to use all content contained in the AAOS Outcomes Material. All rights are reserved.
The American Academy of Orthopaedic Surgeons specifically disclaims warranties, either express or implied, including, but not limited to, implied warranties of merchantability and fitness for a particular purpose, with regard to the AAOS Outcomes

Organizations wishing to sell, sublicense, or otherwise distribute the AAOS Outcomes Material including the questionnaires, scoring algorithms, and normative data as part of their product or service offerings
(whether or not a fee is charged) should contact the American Academy of Orthopaedic
Surgeons for commercial licensing information. Section Three describes the noncommercial license types that are available.
You must read the entire license agreement and agree to all of its conditions and requirements in order to use the AAOS Outcomes Material contained on this web site. You must also complete the on-line non-commercial license application in order to obtain a password and username that will allow you to access the questionnaires, normative data and scoring algorithms.

Single-user, non-commercial license
This category of license is granted to individual users of the AAOS Outcomes Material.
These users are authorized to type the items from the surveys, the scoring algorithms and normative data into a single computer for their own personal and non-commercial use.
Licensee is not authorized to transfer or distribute machine-readable copies to any other person, to use the scoring algorithms or normative data on more than one computer, or to distribute executable programs containing the scoring algorithms or normative data. This license does not entitle the licensee to market, re-sell, sub-license, or otherwise distribute or use the AAOS Outcomes Material including the questionnaires, scoring algorithms, or normative data as part of a product or service offering (whether or not a fee is charged).
This non-commercial license does not allow reproduction or transmission of the survey instrument(s), scoring algorithm(s), and/or normative data on a computer network,
Intranet, Internet server, or Interactive Voice Recognition (IVR) system. This is a royalty-free license.
Multi-user, non-commercial license
This category of license is granted to organizations, hospitals, universities, and government agency etc. who wish to use the AAOS Outcomes Material including the questionnaires, the scoring algorithms and normative data on more than one computer, for non-commercial purposes. Holders of this license are authorized to reproduce the surveys, scoring algorithms and normative data on multiple computers within the same computer network, or restricted-access corporate Intranet or Interactive Voice Response
(IVR) system. This license does not entitle the licensee to market, re-sell, sub-license, or otherwise distribute or use the AAOS Outcomes Material including the questionnaires, scoring algorithms or normative data as part of their product or service offerings (whether or not a fee is charged). This non-commercial license does not allow reproduction or transmission of the survey instrument(s), scoring algorithm(s), and/or normative data on an Internet server. This is a royalty-free license.

Upon reading and agreeing to this license agreement and completing the on-line application, you will be granted a non-commercial license to use the AAOS Outcomes
Material including the questionnaires, the scoring algorithms and normative data. A password and username will be e-mailed to you within 48 hours, which will allow you to access the password protected AAOS Outcomes Material section of the AAOS web site.

By agreeing to this license agreement, you are agreeing to a pay penalty of $5,000 for each misuse of the AAOS Outcomes Material or violation of the non-commercial use license agreement.

This Agreement shall be construed in accordance with and governed by the laws of the
State of Illinois.
