Check list for Information/Communication session □ Agree name to call patient & document in notes □ Check patient understanding of diagnosis and treatment plan/options given by Consultant answering any further questions □ Ensure patient has Key Worker contact information card and leaflet describing the KW/CNS service, knowing how to contact service during the day and out of hours □ Give introduction sheet and complete involving Pre-Assessment clinic (PAC) appointment, bowel preparation (if appropriate) and nutrition drinks etc □ Ensure awareness of bringing a relative / friend to appointments □ Check understanding of any further tests required and provide written information □ Discuss expected post-op recovery care, giving appropriate written information about bowel cancer, their type of surgery including Enhanced Recovery Programme (ERP) information or Oncology treatment □ If having chemoradiotherapy as their first treatment check patient understanding of what it involves and the potential side effects, ensure Oncology appointment booked □ Give details of the ward the patient is likely to be on, giving direct line number □ Inform patient and relative about the option to discuss patient’s progress with consultant after operation whilst on the ward. Nursing or medical staff can arrange. □ Discuss option of referral to cancer welfare benefits adviser, cancer psychologist or social services etc □ Assess any likely issues with discharge home □ Refer patient to stoma care service as appropriate □ Explain discharge information will be given just before discharge on the ward but offer written information at this point too to help with planning family support when patient goes home. Explain about 5 year follow up checks/appointments. □ Ensure written information given about local and national support groups □ Explain about entitlement to free NHS prescriptions □ Offer chemotherapy and radiotherapy booklets and explain about the Chemotherapy Pre-assessment clinic where more information will be available and a chance to answer any further questions □ Ask if covered everything that need to know now, opportunity via telephone helpline □ Use of National Cancer Patient Experience Survey CNS section to ensure all CNS points are covered Clinician Signature: Patient signature: Give a copy to patient and put one copy in patient medical notes.
