If Systolic BP 109 or less, MA to recheck BP with patient
standing.
BP is 100-109, MA to release pt with 3 month follow-up and
route chart to provider with a smart phrase “.vs” in “Routing
Instructions.”
BP is 99 or less, Refer to provider for evaluation. Do not
release the patient.
Direct the patient to arrange for a follow-up office visit in
about 6 months. Release the patient. Route to PCP using
“.vs” in “Routing Instructions”
Direct the patient to arrange for a follow-up office visit in
about 4 months. Release the patient. Route to PCP using
“.vs” in “Routing Instructions”
Direct the patient to arrange for an MA BP recheck (make
the appointment) in about 4 weeks and release the patient.
