Under the proposal both surgeries would continue to operate from their current locations but will be managed by a single partnership. The reason for this proposed merger is to improve the quality of services provided at both surgeries, including access to appointments. By bringing the two practices together we will also achieve better use of the space within each building and be able to share expertise more effectively. Frequently Asked Questions... 1. What is the reason for the merger? We want to merge so that we can offer more services and provide better quality. A merger will help us to share these and other resources for the benefit of our patients. Also... health services are increasingly being delivered in the community rather than in hospitals. So having services closer to where patients live and work is more convenient for patients. We want to be able to deliver more services locally to our patients. Across the two practices we have GPs and Nurses who are specialists in many conditions including skin, heart & chest conditions, addiction and diabetes amongst many others. Patients from both practices will benefit from access to all this expertise across the two sites. 2. What are the main benefits of the proposed merger? We firmly believe that by merging the two practices you, the patient will benefit from: * Improved access to services - longer opening and extended hours * Increased availability of appointments due to more DRs and Nurses. * Increased range of services: e.g. sexual health, addictions, diabetic care, stop smoking service, weight management, heart failure management, counselling services, skin, bone and muscle conditions management and many more... * Wider range of external services being able to provide care from practice premises e.g. Counselling, physiotherapy, citizen’s advice bureau and substance misuse practitioners. * Sharing of skills, Expertise, Experience of all clinicians and patients. * Increased opportunity for same day and booked ahead appointments. * Improving continuity * Better access to telephone consultations * Increased opportunity for patients to see or talk to the doctor or nurse when they want to, and increased practice nurse appointments. 3. Will My Surgery is closing? NO... There are no plans to close either surgery. Patients will be able to carry on under their existing surgery if they want to. 4. Will my GP still be in the same location? YES... GPs will continue to operate from both Lockfield Road and Raynor Road and patients will be able to use the services of both sites. Some clinics may move to Lockfield surgery where there are more adequate facilities. To improve efficiency we may offer some special clinics such as minor surgery, coil fitting and contraceptive implants from one location only. 5. Will I keep the same GP? YES... You will be able to continue seeing the same GP as before. 6. Will the staff stay the same? In the short and medium term... YES... but they will have to work across the two sites. We will work with our team to develop systems for this. 7. Can I be seen at either surgery? YES... Both surgeries will be using the same computer system so your electronic medical records will be available at both locations. You will therefore, be able to access services at either site. 8. What services will be offered at both sites? In addition to the core services we currently provide we will be extending the following access across both sites: * Support to reduce intake of alcohol - Reducing complications from osteoporosis * Better care for people with learning difficulties * Extended hours * Better access to minor surgery procedures - Contraceptive implants, Coil fittings and Smears. * Intermediate smoking cessation for all patients. * NHS Health checks at both sites * Management of patients with prostate cancer in the practice * Better care for patients with: Diabetes, Heart disease, Chronic Obstructive airways disease, Asthma, and other chronic conditions, Better support and care for patients with addictions. 9. Will the current range of services offered by either practice be reduced as a result of the merger? NO... All services currently offered will be maintained. Our goal is to extend the range offered at both sites and will be available to all patients from both practices. 10. When will the proposed merger be completed? Our aim is for the merger to be completed by 1st June 2013 11. What will Lockfield Road and Raynor Road Medical Centre is now known as? Both practices will keep their original names but the partnership will be known as: Lockfield Surgery and Raynor Road Medical practice. 12. Can I get public transport to both sites? YES... Both practices have good public transport links and are within walking distance for many of our patients’ dependant on your choice of attendance at surgery. 13. How can I make an appointment? All patients will have access to both sites and will be able to make an appointment via telephone or in person During our core hours of: 8am – 6.30pm Mondays, Tuesdays Thursdays and Fridays at BOTH surgeries. 8am - 1pm Wednesdays (both sites) 8.00am – 10am Saturdays (Lockfield Site only – pre-bookable appointments only ) 14. Will I be able to get weekend or Evening appointments? YES... Please see above for surgery opening times at both surgeries available for ALL PATIENTS. 15. Is there parking available at BOTH sites? YES... There is a small car park and street parking at Raynor Road Medical Centre and a substantial cark park at Lockfield surgery Both are free of charge but cars are left unattended at owner’s risk. 16. Is there sufficient access for patients with disabilities? YES... Both sites have full access for patients with disabilities 17. What feedback have you had form patients regarding the merger? The Patient Participation Group (PPG) and local council are very positive about the opportunities that the proposed merger will present for the expansion of services to all patients of both practices. It will help to improve access and enable us to provide a much broader range of services to our patients. Please note: The Patient Participation Group meetings will be held at Lockfield Surgery. 18. How can I have my say? We want patients to get involved and have their say about these changes. You can feed back to us via the practice websites and attend events that we as a group will be holding.
