The post holder reports to the General Manager on operational matters and
liaises with clinical and administration staff at all levels throughout the hospital
and with the Clinical Indemnity Scheme of the State Claims Agency.

Mayo General Hospital
Mayo General Hospitals has 306 beds; its aim is to deliver high quality and
equitable care for all its patients, in a safe and secure environment, and to
achieve excellence in clinical practice, teaching, training and research. The
hospital has an ongoing modernisation programme and has recently introduced
new computer based systems such as NIMIS system. MGH provides a
comprehensive range of services to emergency and elective patients.
The post holder is professionally accountable to the Director of Nursing or
designate.
Purpose of the
Post
The Quality and Patient Safety (Q&PS) Manager will manage the implementation
of quality and patient safety programmes to include:








Risk Management,
Quality Improvement Programmes,
Standards including oversight of Policies, Procedures and Guidelines,
Governance,
Complaints and Advocacy,
User Involvement,
Audit,
Management of Healthcare Acquired Infections.
The Q&PS Manager will maintain Mayo General Hospital’s Clinical Risk
Management Programme, and be involved in the implementation of the STARS
Incident Reporting and Claims Management system.
He/she will also be involved in the continuous development of quality healthcare
delivery at Mayo General and serve on various hospital committees for that
purpose.
He/she will undertake Quality, Patient Safety and Clinical Risk awareness
programmes for all staff as appropriate including each intake of NCHDs.

Principal Duties
and
Responsibilities

To have primary responsibility for putting Quality and Risk systems in
place in the hospital to reassure the Hospital’s General Manager that
patients are safe and that a quality service is being provided.

To manage the planning and development of an integrated and
comprehensive healthcare Quality and Patient Safety management
programme in the hospital.

To manage the implementation of this integrated and comprehensive
Quality and Patient Safety programme, to include Risk Management,
Quality Improvement programmes, Standards (PPGs oversight),
Governance, Advocacy, User Involvement, Complaints Analysis, Audit, &
HCAI management.

To manage the strategic, advisory and monitoring functions of the
healthcare risk management service for the continuous improvement of
this integrated and comprehensive healthcare Q&PS Management
programme. This will include the rigorous implementation of the HSEs
incident investigation and reporting policies and protocols, in addition to
audit, support and advisory work for ongoing preventative and reactive
healthcare risk management work.

To oversee the incident reporting and investigation procedure throughout
the hospital.

To oversee the maintenance of an incident / near miss database system.
To maintain good working relationships with medical, nursing and other
personnel in managing the clinical incident reporting system and to work
with clinical staff to ensure that all available data is integrated to promote
and improve patient care.

To oversee collation of data provided by this incident / near miss
database to provide useful management information to guide the future
work of the healthcare Q&PS Management service to focus on risk
priority areas.

To oversee the feedback of useful information from this incident / near
miss data base system to local managers to enhance local risk
management initiatives. To evaluate incident data as to severity,
frequency, location and remedial action required.

To manage the delivery of training and education on healthcare Quality,
Patient Safety and Clinical Risk issues, deliver presentations and
promote an effective communication network.

Facilitate the continuance of a culture of risk awareness among all
hospital staff and provide support to departmental risk awareness teams.

To manage the implementation and evaluation of specific patient safety
and quality projects in association with service providers, audit staff and
others as required.

To ensure the role out and implementation of all standards and related
monitoring to ensure that standards are being implemented.

To facilitate the development of protocols for safe patient care in
accordance with recognised standards.


Professional Guidelines and to keep up to date with development in best
practice in Quality, Patient Safety and Clinical Risk Management.

To recommend to the General Manager changes in policies, procedures,
staffing levels, skill-mix and education programme required for
compliance purposes.

To be involved in the analysis of claims for damages and compensation
including the HSE, insurers, and medical indemnity societies and to
provide advice and support, where competent to do so, to staff involved
in litigation claims and complaints.

To be involved in the analysis of patient complaints relating to clinical
care and to advise the General Manager on best practice.

To have involvement in ensuring that statutory obligations in regard to
Health and Safety are complied with.

To promote best practice in relation to healthcare records and liase with
Medical Records Officer in doing so.

To manage and supervise staff as required.

Qualifications and/
or experience
The reform programme outlined for the Health Services may impact on this
role and as structures change the job description may be reviewed.
Candidates must
Be registered in the Relevant Division of the Register of Nurses kept by An Bord
Altranais or be entitled to be so registered
And
Have at least 5 years post registration experience of which 2 must be in the
speciality or related area
And
Have the clinical, managerial and administrative capacity to properly discharge
the functions of the role
And
Demonstrate evidence of continuing professional development at the
appropriate level.
Age
Age restrictions shall only apply to a candidate where he/she is not classified as
a new entrant (within the meaning of the Public Service Superannuation Act,
2004). A candidate who is not classified as a new entrant must be under 65
years of age.
Health
A candidate for and any person holding the office must be fully competent and
capable of undertaking the duties attached to the office and be in a state of
health such as would indicate a reasonable prospect of ability to render regular
and efficient service.

Terms and Conditions of Employment
Clinical Nurse Manager 3-Quality and Patient Safety Manager
Tenure
The appointment is whole-time, permanent and pensionable.
Appointment as an employee of the Health Service Executive is governed
by the Health Act 2004 and the Public Service Management (Recruitment
and Appointment) Act 2004.
Remuneration
The Salary scale for the post is as below (correct at 01.01.2010):
€54,336 - €55,430 - €58,119 - €59,228 - €60,382 - €61,491
Working Week
The standard working week applying to the post will be confirmed by NRS
at expression of interest stage.
Annual Leave
The annual leave associated with the post is 25 days per annum, 26 days
per annum for greater than 5 years service and 28 days per annum for
greater than 10 years service.
This will be confirmed by NRS at expression of interest stage.
Superannuation
This is a pensionable position within the HSE. The successful candidate
will upon appointment become a member of the appropriate pension
scheme. Please be advised that pension scheme membership will be
notified within the contract of employment.
Members of pre-existing pension schemes who transferred to the HSE on
1st January 2005 pursuant to Section 60 of the Health Act 2004 are
entitled to superannuation benefit terms under the HSE Scheme which
are no less favourable to those to which they were entitled at 31st
December 2004.
Probation
Every permanent appointment of a person who is not already a permanent
officer of the Health Service Executive or of a Local Authority shall be
subject to a probationary period of 12 months as stipulated in the
Department of Health Circular No.10/71.

As this post is one of those designated under the Protection of Persons
Reporting Child Abuse Act 1998, appointment to this post appoints one as a
designated officer in accordance with Section 2 of the Act. You will remain
