BONE DENSITY TESTING INSTRUCTIONS
(Diagnostic Test for Osteoporosis)
To better serve you, following are a few instructions you will need in order
to have the Bone Density Testing that your physician has referred you for.
Please arrive at our office 15 minutes prior to your scheduled appointment time.
Please fill out the attached Patient Information Form for the office of Raj Dalal, M.D.
Bring this form at the time of visit for Bone Density testing.
Bring with you to the appointment:
Driver’s License.
All Insurance Cards - we cannot do test if these are not brought in.
List of all current prescription medications, vitamins and over the counter medications.
Any paperwork your referring physician has given you for the test.
Please be sure to check with your insurance company prior to the appointment to be
sure it is a covered charge. You will need to let them know this is an outpatient
diagnostic test (DEXA scan) to check for osteoporosis. Also be sure to find out if you
have met your deductible and if not how much has been met. You will be expected to
pay your portion of the test at the time of your appointment. The amount will be
determined by your insurance and deductible. We accept cash, check, MasterCard
and Visa.
Do not wear anything that has metal such as: zippers, under wire bra or bra that opens
in the front, metal buttons, metal paint or metal threads on your shirt.
All results will be sent to the physician that referred you. Please call that office two
weeks after testing in order to be given your results.
