Please complete the attached order form: Circle the dollar amount for those months
your child would LIKE to receive hot lunch. Place an “X” over any month’s menu
your child will NOT be ordering. If necessary, the committee reserves the right to
replace menu items with a similar district-approved item.
* If you are able to volunteer for any or all of the above dates, please fill out the parent
volunteer form below and return it with your child’s hot lunch form. Your help is greatly
appreciated!
Return the attached hot lunch order form, the parent volunteer form below, and your
check (payable to Summit Hill School District 161) in an envelope with your
child’s name, grade, and teacher listed on the bottom no later than Friday,
September 7, 2012. Checks only, please!

Parent Volunteer Form
If you are interested in volunteering for any or all of the dates above, please fill out the
information below. You will be contacted by e-mail regarding your availability prior to
each hot lunch date. Phone calls will be placed only if necessary.
Volunteers should sign in at the office (picture ID required) and report to the lunchroom
kitchen. 5th Grade volunteer from 11:00 am to 12:00 am and 6th grade from 11:50 am to
12:50 pm. Total volunteer time should not exceed 60 minutes. Siblings are not allowed.
