Patient Centered Medical Home (PCMH) Reception On June 11, 2012, The Medical Society of Delaware and Blue Cross Blue Shield of Delaware hosted a reception for multiple stakeholders in the health care community to celebrate the launch of Delaware’s first patient centered medical home (PCMH) pilot. The pilot, called Patients First in the First State (often referred to as “Patients First”), launched in May.
