partnership and develop integrated models of care

• Work jointly to promote and redesign services based on
integrated models and build community resilience
• Work jointly on and agree a service offer for Oldham
• Work together to deliver system change in Oldham and lead
transformation

• Cluster-based integrated health and social care teams wrapped
around the Primary Care Medical Home
• Flexible and responsive to population needs
• Provide high quality, personalised care with a focus on patients
with long term conditions
• Aim to reduce emergency admissions, improve patient
experience and social and health outcomes
• Fully in place by June 2014
• New cluster performance and development leads being
recruited

Single point of access
• Urgent access to all community healthcare services is available
through a Single Point of Access
• Staffed by a multi-disciplinary team of health professionals
• Can also be used for general enquiries from GPs and other
professionals, and patients and carers.
• Onward referral to social care can also be made
