Little League® Tournament A-Z Tournament Affidavits Tournament Eligibility Affidavit • • League President is responsible for reviewing and certifying birth records and residence documents. Tournament team to have in its possession: – Eligibility Affidavit; – Map of actual boundaries of the league with locations noted for residences of the parent or legal guardian or location of the school for every player named on the affidavit; – Tournament verification form for each player; – Copies (originals not required) of records used to verify date of birth; – Three or more documents to determine residency of the parents or legal guardian dated on or in force between February 1, 2013 (previous year) and February 1, 2014 (current year); or a document to support school attendance for every player named on the affidavit Waivers (i.e. II(d), IV (h), Charter Committee, etc.) Tournament Eligibility Affidavit Important: • • • • Alternates are not authorized. They shall not accompany the team and shall not be listed on the affidavit. Affidavit must be certified by the District Administrator or his/her designated appointee. Each Tournament Team must have twelve (12) eligible players for the District Administrator to certify. Exception: A district administrator may certify the Eligibility Affidavit for those teams that provide a justifiable reason for not having twelve (12) players. President and Player Agent Certify: •They understand the rules and regulations pertaining to participants. •That names, dates of birth and residence or school attendance of those listed are true and correct. •In the event of a controversy, agree to accept the decision of the Charter/Tournament Committee as final and binding. Regular Season Team Code •Letter under Code corresponds with each team. •Team noted must be in proper division of the league or combination approved by Regional Director. Manager / Coach Replacement Notes: • Temporary Replacements (single game only) of a manager/coach should not be entered. • Replacement spaces are to be used for permanent replacements only. • Once replaced, the manager/coach cannot return to the roster. Baseball Tournament Pitcher’s Record Notes • Each pitcher who delivers one or more pitches to a batter must be entered in every game. • Completed after each game. • Should be reviewed for accuracy by team manager. Softball Tournament Pitcher’s Record Notes • Each pitcher who delivers one or more pitches to a batter must be entered in every game. • Completed after each game. • Should be reviewed for accuracy by team manager.
